Abstract
The purpose of this study was to reduce 30 day Heart Failure readmissions utilizing telehealth as part of the continuum of care, transitioning patients to the outpatient setting in a rural community with a focus on self management and education in dealing with a chronic disease. Our facility is a 214-bed regional referral center located in a rural county on northern Lake Michigan. Our service area comprises three counties with referrals from approximately 115 miles in each direction of Michigan's northern Lower Peninsula and eastern Upper Peninsula.
