Abstract

Video Objective Demonstrate the treatment of a Congenital uterine malformation (UB2-C2), hysteroscopically in conjunction with laparoscopic revision. Setting We present the case of a 44-year-old woman with abnormal uterine bleeding. At clinical assessment she refers >8 days of transvaginal bleeding; and less than 15 days without bleeding. Interventions We used a 5 mm LIGASURE BLUNT-TIP, to dissect the vaginal septum; evidencing two cervixes, proceeding to collocate a uterine manipulator. Then proceeding to realize the laparoscopic revision followed by a total laparoscopic hysterectomy. In general, an incidence of around 4% of all Congenital uterine malformations is reported. (1) Mullerian alteration of septate uterus with duplication of the cervix and longitudinal vaginal septum was first described by Mc Bean and Bramsted in 1994. (2) In the ESHRE / ESGE classification, the patient enters the U2b category, due to having two cavities completely separated by a septum, coexisting with two complete cervixes. (1) Conclusion Congenital uterine malformations are responsible for a variety of symptoms and health problems depending on the anatomical defect of the uterus and/or cervix. Further investigation is required to establish unanimous guidelines for the individualized and suitable invasive or not management of each patient.
