Abstract
Introduction: In May 2010, Alberta Health Services (AHS), building on work previously developed within 12 distinct health entities, set out to design a comprehensive service delivery model that would address obesity prevention and management across the continuum of care, across the lifespan, and across the province. As an integral part of this business planning, efforts were focused on translating high-level objectives into management action, and linked performance and evaluation measures. We report on lessons learned, and the resultant Alberta Obesity Program Logic Model, and its output and outcome indicators.
