Medical history was significant for morbid obesity s/p Roux-en-Y bypass surgery, end-stage renal failure on hemodialysis, substance abuse, congestive heart failure, and depression. Medications included duloxetine, apixaban, and pantoprazole. On physical exam, the patient had multiple tense bullae with surrounding erythema and scattered crusts on her bilateral dorsal hands.
