AMR is triggered by anti-donor blood group A/B antibodies (Abs), which might bind to blood group A/B-antigen on the endothelium of the graft. The advent of new immunosuppressive agents and advances in immunosuppressive therapy have markedly improved outcomes ABO-I KTx. It is well known that various strategies have been performed to remove or to reduce anti-blood group Abs. However, blood group antigen-blocking treatments have not been tried in clinical application. In this study, we tried to indentify a peptide sequence which specifically binds to blood group antigen. METHODS We screened a blood group A- or B-antigen binding 7-mer peptide by T7 phage random peptide library display screening system. Next, we investigated whether A or B-antigen binding peptide can inhibit hemagglutination (HA) of blood group A/B red blood cells (RBCs) by anti-A or -B antibody. Furthermore, we investigated whether A/B-antigen binding peptide can bind to blood group A- or B-antigen on the paraffin-embedded and frozen kidney tissue section. RESULTS We identified eight 7-mer peptides which binds to blood group A (ASNKRPR, SRAPKPR, ARARKTG and RPRNPNK), blood group B (GKTRSK, ALRTRSK, SRSSTRK) or both blood group antigen (RMSRKLP) respectively. Blood group A/B binding peptides coated RBCs were inhibited HA activity by anti-A or B antibody. Furthermore, these peptides also bind to A/B-antigen on the microvascular, peritubular and glomerular endotherium of kidney. Binding pattern of peptide on kidney tissue was very similar to that of anti-blood group antibody. CONCLUSIONS These data indicating that blood group A/B-antigen on RBCs and on kidney tissues may neutralize by A- or B-antigen binding peptide.
