Abstract
症例は75歳の女性で，上行結腸癌にて1987年8月右半結腸切除術を施行後，1988年，1989年に左鎖骨上リンパ節転移が出現し，それぞれ郭清術を施行した．以後約14年間再発を認めなかったが，2003年11月左鎖骨上窩および左腋窩のリンパ節再発を認めたため，従来の化学療法を5年以上行った．2009年8月で病勢制御不良となったため，bevacizumab+mFOLFOX6を施行すると転移巣が著明に縮小し，2010年3月左鎖骨上窩～腋窩を郭清した．その後，隣接した少数のリンパ節転移を2回追加郭清したが，3年8か月間再発を認めず，化学療法も5年間施行していない．大腸癌遠隔リンパ節転移について集学的治療の有効性を示す報告は少ないが，自験例のような遠隔リンパ節転移を来し長期担癌状態の後に根治的郭清術を施行した症例はまれであり，同様の報告は他に認めなかった．
