Abstract

The New York City Health and Hospitals Corporation (HHC) is the country's largest public hospital system, providing primary care services to nearly 50,000 adolescent patients annually. HHC started the Teen Health Improvement Program (THIP) in 2012 to improve population health outcomes among adolescent patients by ensuring access to high quality health care services. Most adolescent patients at HHC are seen in pediatric clinics by general pediatricians. Clinicians and staff in these settings may require support to better serve patients as they transition from childhood to adolescence to young adulthood.
