METHODS One hundred eighty two patients with complicated impacted upper ureteral stones above the level of L4 were randomly divided into 2 groups. Group 1 comprised 91 cases that were treated with transurethral ureteroscopic lithotripsy (URL). Group 2 comprised 91 cases that were treated with MPCNL with the aid of a patented stone-breaking and clearance system (PMPCNL). The patients received postsurgical shock wave lithotripsy (SWL) when necessary. The operative time, rate of stone clearance, operative complication markers, including amount of intraoperative bleeding, rate of postoperative fever, and cost of treatment, were compared. RESULTS There was a significantly shorter operative time, a higher rate of stone clearance, less need of postoperative SWL therapy, and a lower rate of postoperative fever in Group 2 compared to Group 1. However, there was a significantly higher cost of treatment and a higher amount of intraoperative bleeding in Group 2. CONCLUSIONS We think MPCNL with the aid of the patented stone-breaking and clearance system could be the first choice in treating complicated impacted upper ureteral stones above the level of L4.
