However, it is not well known whether low percentage of PBC can predict lower PSA recurrence risk. We investigated whether low percentage of PBC has potential to predict favorable outcome in PSA recurrence in cT1c prostate cancer. METHODS The records of 1268 consecutive cases with localized prostate cancer who were treated radical prostatectomy without neoadjuvant therapy from Jan 2001 to Dec 2009 were retrospectively reviewed. We identified 681 cases with T1c cases. They were divided into 3 groups according to percentage of PBC; low percentage PBC group who had ≤10% PBC, intermediate PBC group who had 11–74% PBC, and high PBC group who had 75-100% PBC. Preoperative factors including age, initial PSA, percentage of PBC, biopsy GS, D'Amico risk were compared among the 3 groups. The PSA recurrence free survival was estimated by Kaplan-Meier method, and multivariate analysis was used to analyze the predicting pT3, and PSA recurrence. RESULTS Of 681 patients, 164 were included in low PBC group, 503 were in intermediate PBC group and 14 were in high PBC group. Median follow-up periods were 47 months. There were no significant differences in age, iPSA, biopsy Gleason Scores among the groups. Number of biopsy, percentage of PBC and incidence of pT3 were significant higher in intermediate and high PBC group compared with low PBC group. PSA recurrence free survival was significantly low in high PBC group. However, there was no significant difference between low PBC and intermediate PBC groups. Multivariate analysis showed low percentage of PBC was independent factor for pathological organ confinement, while it did not have significant association with low risk of PSA recurrence.
