The mechanical traction phenomenon is well known and has potential application in the corporeal fibrosis, tissue retraction and shortening. The aim of this pilot study was to evaluate the efficacy and safety of the use of the penile extender device (PED) in the initial phase of PD. METHODS From Jan 2009 to Sept 2010, 45 pat. with clinical diagnosis of PD in acute phase (evolution period of less than 6 months and pain at rest or with erection) were invited to participate in this prospective pilot study. The following variables were collected: age (years), ° of deviation measured by auto photos (Kelami test) an with ICI, length-girth in stretching and flaccidity status, IIEF-SHIM version, overall satisfaction (OS) 1–100%, pain in erection and flaccid by visual-analogue scale (VAS). Patients were instructed and properly trained to use PED at least 6 h and no more than 8, preventing its use during sleep. Auto photos during erection were carried after 1, 3, 6 and 9 m, comparing the angle of deviation and pat. satisfaction was reassessed at each visit. The Chi-square tests was used to compare discrete factors, t-tests for continuous factors. RESULTS Mean age was 47 years (39–72). The mean curvature was 20–-55° (10°–80°) measured by ICI. The average treatment period was 6 months (4–12 months) the mean reduction in the angle of curvature, measured by Kelami was 45% (15–70%). The average length stretch was 12.4 cm (9–14.5) and flaccidity of 9.5 cm (8.5–12.5). The average length increase was 1.9 cm in stretching and 1.2cm in flaccidity (p>0.05). Mean pre-treatment VAS score was 5.5 and 3.5 at 6 m. Four patients developed erythema in balanopreputial groove that did not require any discontinuation. Mean pre-treatment IIEF-SHIM was (20) and final (after 9 m) of 26.5 (p<0.05). The average OS was 85% (50–90%). Five (14%) out of the 35 pat. included in the study underwent surgery for persisting a curvature > 45° and difficulty for penetration after 9 months of treatment. These patients had a baseline curvature > 60°. CONCLUSIONS In our initial and limited experience, PED seems a safe and effective treatment in terms of overall satisfation, penile curvature as well as pain reduction in the acute phase of PD. After 6 months, statistically sexual function improvement is shown (p<0.05).
