Participating centers undergo a comprehensive approval process necessitating reaccreditation every five years. In this study, we examine the effect of residency/fellowship teaching status and hospital volume (HV) on perioperative RC outcomes. METHODS Within the Nationwide Inpatient Sample (NIS), we focused on RC performed between 2007 and 2010. We tested the rates of in-hospital mortality, as well as intraoperative and postoperative complications, stratified according to teaching status. Multivariable logistic regression analyses further adjusted for confounding factors. Hospitals were considered residency teaching institutions if they had an American Medical Association (AMA)-approved residency program, are a member of the Council of Teaching Hospitals or had a ratio of full-time equivalent interns and residents to beds of 0.25 or higher. Hospitals with a SUO fellowship were recognized as fellowship teaching institutions. RESULTS Overall, 19462 patients after RC were identified. Of these, 4546 cases were performed at non-teaching institutions, 9462 at residency teaching institutions and 5454 at fellowship teaching institutions. Patients treated at residency/fellowship teaching institutions were more likely to hold private insurance and healthier relative to those treated at non-teaching institutions. In multivariable analyses, patients treated at residency (OR=0.74, p<0.001) and fellowship (OR=0.66, p<0.001) teaching institutions were less likely to experience a postoperative complication relative to patients treated at non-teaching institutions. CONCLUSIONS On average, patients treated at residency and fellowship teaching institutions are less likely to experience a postoperative complication. Moreover, postoperative complication rates were lower at fellowship teaching institutions, relative to residency teaching institutions, despite adjustment for potential confounders.
