Obese patients with benign prostate hyperplasia (BPH) may have lower free PSA as well as PSA due to hemodilution. Lower fPSA may falsely affect estimation of TPV. The aim of this study was to determine fPSA mass as the absolute amount of fPSA protein secreted into circulation, and to evaluate its usefulness in the prediction of TPV. METHODS Subjects were 1788 men with PSA up to 10 ng/ml, including 1657 men who underwent a transrectal ultrasound (TRUS)-guided extended prostate biopsy and were proven to have BPH. Body surface area (BSA), plasma volume, PSA mass and fPSA mass were estimated by means of the following established formulae: BSA (m2) = body weight (kg)0.425 × height (m)0.72 × 0.007184, plasma volume (L) = BSA (m2) × 1.670, PSA mass (μg) = serum PSA (ng/ml) × plasma volume (L) and fPSA mass (μg) = serum fPSA (ng/ml) × plasma volume (L). The association between PSA, PSA mass, fPSA or fPSA mass and TRUS-measured TPV were evaluated by Pearson′s correlation coefficient (γ), linear regression analyses and receiver operating characteristic (ROC) curves. RESULTS Both PSA and fPSA had significant and positive associations with age and TPV with significant and negative associations with plasma volume. fPSA showed a strongest and significant correlation with TPV, followed by fPSA, PSA mass and PSA with γ of 0.52, 0.50, 0.28 and 0.26. The ROC curves showed that fPSA mass (area under the curve [(AUC] = 0.795) outperformed fPSA (AUC = 0.780), PSA mass (AUC = 0.707) or PSA (AUC = 0.690) did at estimating TPV cutoff values of 30 ml. CONCLUSIONS This is the first study showing the performance of fPSA mass at estimating TPV.
