The aim of the present study is to evaluate the incidence, risk factors and oncological outcomes of positive surgical margins (PSM) after NSS. METHODS We identified 509 patients in our institute database that underwent NSS. The association between PSM and demographic (age, gender, year of surgery), pathological (tumor size, histologic subtype, clinical stage), surgical (type of surgery, duration, warm ischemia time) variables and oncological outcome (local recurrence, metastatic progression, disease specific mortality). RESULTS PSM was found in 4.6% of cases. PSM wa associated with histological subtype (p<0.05) and inverse related with year of surgery (p=0.03). no association was identified between PSM and age, gender, tumor size, stage, duration of operation and warm ischemia time. No case of PSM was identified during 2010-2012. No correlation was identified between PSM and oncological outcomes. CONCLUSIONS The incidence of PSM after NSS is low. We found a correlation between histological subtype and PSM. Furthermore, there is a clear and consistent decline in PSM incidence in the last decade. We could not find any connection between PSM and oncological outcome.
