Abstract

Capsule endoscopy has been approved as a priority for detection of intestinal abnormalities. But it is limited by the poor image quality and incomplete small bowel transit. Some studies have shown that administering polyethylene glycol or water during the capsule endoscopy yielded an improved completion rate and image quality. This prospective randomized controlled trial was designed to evaluate the value of diving method for capsule endoscopy in examination of small intestinal disease, providing a more reasonable bowel preparation protocol for capsule endoscopy. A total of 140 patients were prospectively recruited. The patients were randomized into two groups by the random number table: the control group (conventional preparation) and the diving group. For the diving group, except with conventional preparation, if the capsule had reached the small bowel ascertained by a real-time viewer, 500ml of water was administered; then 500ml of water was administered every hour until the capsule ran out of power or went into the colon. The small bowel transit time (SBTT), quality of the capsule endoscopic image and lesion detection rate were analyzed in two groups. The scoring system for image quality (Fig.1) was described by Hosono et al. The completion rate of the diving group was significantly higher than that of the control group [92.19% (59/64) vs 76.32% (58/76) ].The SBTT of the diving group and the control group were: 261.14±124.72 min vs 299.34±96.40 min (P=0.067). For the quality of the capsule endoscopic image, scores of the proximal third, middle third and distal third of the small bowel in the diving group and the control group respectively were (Table 1): 3.47±0.60 vs 3.11±0.63 (P=0.007), 3.24±0.59 vs 2.78±0.74 (P=0.002) and 2.71±0.94 vs 2.48±0.78 (P=0.254). The lesion detection rate was no significant difference [69.49% (41/59) vs 74.14% (43/58) ].
