Roswell Park Cancer Institute (RPCI) located in Buffalo, NY was the nation's first hospital dedicated to cancer patients & research. Its establishment in 1898 against the odds of the prevailing administration is dedicated to a Surgeon, Dr Roswell Park. Since its inception, RPCI as an institute & its physicians has contributed immensely into the field of cancer medicine & urology. METHODS Research material was obtained from the archives of RPCI, in particular a publication by Dr E Mirand. Material was also obtained by personal communications and literature search. RESULTS Being concerned about the lack of research & management of cancer, Dr Park was initially denied financial assistance to set up a research facility from the NY Governor Frank Black in 1897. Further lobbying finally realised his dream & he started the work on building the first ever cancer research laboratory in the whole of United States in 1898. From a 3 room facility back then to a multi-specialty cancer hospital today, RPCI has made significant contributions to cancer medicine. Dr Gerald Murphy, an outstanding Urologist was the director of RPCI from 1970 to 1985. Along with Dr Ming Chu, he developed the PSA test. Jerry was also instrumental in passage of the National Cancer Act in 1971. He contributed over 1,200 publications and was editor of The Prostate for 21 years. In 1972, Dr T Dougherty devised the first commercial photo-dynamic therapy which is now used in bladder & prostate cancer. Dr Holland's group was one of the first to try cisPlatin as a chemotherapy agent for testicular tumour in the ‘70s. SELECT trial, the largest chemoprevention strategies against prostate cancer was initiated at RPCI by Dr E Levine. Dr Robert Huben (1950-2011), served for more than three decades in Urology and his contribution was recognized in 2006 with the naming of the Robert P. Huben Endowed Professorship. Today, RPCI leads the International Robotic Cystectomy Consortium, which is the largest database of patients undergoing robotic cystectomy in the world. RPCI along with University of Buffalo designed RoSS, a robotic surgery simulator, which was only second of its type in the market. CONCLUSIONS RPCI, being the first cancer hospital in the country, set an example for establishment of similar centres across the US and indeed the globe.
