Abstract
症例は80歳，男性．進行性前立腺癌（T3N1M1 Stage IV）にてホルモン療法開始した．3年9カ月経過後右乳房腫瘤を認め右乳房部分切除術施行し充実腺管癌の病理診断であった．その後，経過観察をしていたところ11カ月後に左乳房腫瘤出現し左乳腺腫瘍摘出術を施行し充実腺管癌の病理診断であった．本症例は，当初，両側乳腺腫瘍ともに浸潤性乳管癌（充実腺管癌）との病理組織報告であったが，追加検索の結果，両腫瘍ともH.E.染色にて乳管内に腫瘍病変を確認することはできなかったこと，免疫染色にてER陰性，PgR陰性，GCDFP-15陰性，CK7陰性，CK20陰性であり原発性乳癌は否定的で，その一方PSA陽性という結果であった．臨床経過・画像所見・病理所見を総合的に検討し，前立腺癌両側乳腺転移との診断に至った．今回原発性乳癌との鑑別を要した前立腺癌両側乳腺転移の1例を経験したので，文献的考察も加え報告する．
