Abstract
本邦における心内異物はほとんどが人為的，医原性であり，飛来性の異物によるものは稀である．今回われわれは，手術により摘出し救命し得た飛来性心内異物の症例を経験した．［症例］27歳男性．エア釘打ち機を使って作業をしていたところ，釘が弾かれ破片が頸部に当たって受傷した．同日近医を受診したが異常は指摘されなかった．2カ月後胸部X線写真で心陰影に重なる異常陰影を指摘された．CT検査上右室内に釘のような異物影を認めた．明確な受傷機転は不明であったが，2カ月前の外傷に関連した飛来性心内異物と診断し，緊急手術を行った．人工心肺補助下に，経右房的に右室から異物を摘出した．［考察および結語］本症例は屋外での受傷であり，感染性心内膜炎，敗血症の合併などが懸念されたが，治療が奏功し術後経過は良好であった．破片が高速で飛散し受傷した外傷では，体内への異物侵入，心内異物の可能性も考慮に入れ，可能な限り全身，特に胸腹部の検索を行っておくべきであると考えられた．
