Abstract
症例は57歳，男性．他院で胆嚢総胆管結石症の診断で内視鏡的乳頭切開+砕石後，腹腔鏡下胆嚢摘出術（以下LC）施行．3年7カ月後の臍下部腫瘤生検で腺癌の結果，転移性腹壁腫瘍として当科紹介．8×8cm腹壁腫瘍に対して腹壁全層切除施行，左大腿筋膜長筋皮弁で再建．肝門部腫瘍や腹膜播種を認めなかった．病理所見で高分化型管状腺癌，CK7（+），CK20（-），CEA（+），CA19-9（+）であった．胆嚢標本の再切り出し病理でも悪性所見を認めなかったが，LC後創膿瘍の経過，腹壁腫瘍所見から早期胆嚢癌のポート部再発（以下PSR）と診断した．3年7カ月後に腹直筋断端再発し，以後2回の腹壁切除を施行したが，腹膜播種・恥骨転移をきたし永眠された．LCから11年，初回腹壁切除から7年2カ月後であった．PSRの多くは予後不良だが，腹膜播種が併発していない症例では腹壁切除により長期生存の可能性がある．
