Abstract
神経鞘腫は末梢神経のSchwann細胞に由来する腫瘍で，主に四肢および頭頸部に好発するが後腹膜にも発生することがまれにある．今回，われわれは解剖学的な位置関係のため術前診断が困難であった，上腸間膜動脈周囲神経叢に発生した神経鞘腫の1例を経験したので報告する．症例は54歳，男性．検診の腹部超音波検査で膵背側に腫瘤性病変を指摘され当科紹介受診となった．腹部CT検査では上腸間膜動脈の左側に境界明瞭な25mmの腫瘤像を認め，膵との境界は比較的明瞭であった．PET検査で異常集積を認めたが良悪性の鑑別は困難であり悪性の可能性を否定できず，GIMT（gastrointestinal mesenchymal tumor）の診断で手術を施行した．腫瘍は上腸間膜動脈周囲神経叢と連続しており，神経原性腫瘍を疑い摘出術を行った．病理組織学的検査でAntoni A型優位の神経鞘腫と診断された．
