Abstract
腹腔鏡下鼠径ヘルニア修復術（totally extraperitoneal inguinal hernia repair:TEP法）後，腹膜欠損部に小腸が嵌入し，イレウスを発症した1例を経験した．患者は84歳，男性．右鼠径ヘルニアに対し，腹腔鏡下鼠径ヘルニア修復術を施行した．後3日目にイレウスを発症し，CT検査にて腹膜前腔に腸管の脱出を認め，イレウス管を挿入し，減圧を行った．術後4日目に腹痛を認め，症状改善ないため再手術を行った．鏡視下で腹腔内を観察したところ，腹膜欠損部より腹膜前腔に小腸が嵌入し，一部に軽度の血流障害を認めた．腹膜欠損部は5cm程度に拡大しており，腹膜を縫合閉鎖し，手術を終了した．腹膜欠損部は当初は1cm程度であったが，本症例のように術後腸管が嵌入する可能性があるため，予め腹膜欠損部が生じた場合は可及的に縫合閉鎖する必要があると考えられた．
