Abstract
症例は63歳,女性.左前腕のしびれ感と歩行障害で発症した.入院後,左不全片麻痺の増悪と右半身T-1レベル以下の温痛覚障害が認められた.頸椎MRI拡散強調像で下位頸髄に高信号域がみられ,T2強調画像にてC-7レベルで頸髄腹側に左優位の高信号域が認められた.血清学的検査および髄液検査から,炎症性疾患や脱髄性疾患は否定的であった.片側優位の前脊髄動脈症候群を疑い,副腎皮質ステロイド,高浸透圧溶液を投与した.温痛覚障害は残存したが左不全片麻痺は徐々に改善し,独歩退院となった.片側性前脊髄動脈症候群は急性発症する脊髄疾患のなかでもきわめて稀であり,MRIによる早期の診断と治療開始が重要といえる.
