Abstract
膵十二指腸動脈瘤は腹腔内動脈瘤中2%と非常に稀な病態である．内視鏡的総胆管結石切石後に前上膵十二指腸動脈瘤破裂を発症した1例を経験した．症例は74歳，男性．主訴は心窩部痛，黄疸．入院時採血所見ではWBC 7400/mm3，T-Bil 5.3mg/dl，P型amylase 33U/l，CRP 5.0mg/dlであり，腹部造影CTで下部胆管に結石を疑ったため第2病日ERCPを施行した．EST小切開を施行し泥状の結石を除去後，経鼻胆管ドレナージtubeを留置．第3病日，出血性ショック状態となり腹部造影CTを施行したところERCP後の軽症膵炎及び後腹膜血腫を認めた．十二指腸動脈瘤破裂の可能性も考慮し血管造影を施行した．破裂部位に対しプラチナマイクロコイルを用い塞栓．以後十二指腸狭窄も合併せず第28病日に退院した．入院時の腹部造影CTでは動脈瘤の所見はなくERCP後膵炎が前上膵十二指腸動脈瘤破裂の誘因になった可能性が考えられた．
