Abstract
気管支動脈蔓状血管腫は，気管支動脈が蔓状に屈曲，拡張，蛇行，増生し，喀血の原因となる稀な疾患である．治療として肺葉切除・気管支動脈結紮などがあるが，近年では低侵襲な気管支動脈塞栓が選択されることが多い．今回，気管支蔓状血管腫による喀血に対して気管支動脈塞栓を行うも再発を繰り返したため，胸腔鏡下気管支動脈結紮を施行した症例を報告する．患者は67歳，女性．喀血の加療目的で当科に紹介された．気管支動脈造影検査にて右気管支動脈の拡張，蛇行および肺動脈との短絡路を認め，気管支動脈蔓状血管腫と診断し，気管支動脈塞栓術を行った．その後も再発を繰り返したため，胸腔鏡下気管支動脈結紮を施行した．術後は再発を認めていない．本疾患に対する気管支動脈塞栓術は低侵襲で有効な治療法として知られているが，再開通による喀血を繰り返すような症例に対しては手術を考慮する必要がある．
