Abstract
症例は35歳，男性．検診の上部消化管造影検査で噴門部に圧排所見を指摘され，精査加療目的で当院紹介．CTで膵尾部に径5cmの嚢胞性病変が存在した．MRIでは膵尾部より頭側方向へ突出する壁の薄い単房性嚢胞性腫瘤で，内部はT1WIでlow intensity，T2WIでhigh intensityを呈した．MRCPでは尾側膵管は腫瘤の圧排のため描出されず，EUSで腫瘤内部は比較的均一な粘液像と判断され，膵粘液性嚢胞腫瘍と診断した．手術所見では腫瘤は胃体上部後壁から発生しており，胃局所切除術を施行した．切除標本の割面は，単房性嚢胞の内容はゼリー状であった．病理組織検査で嚢胞壁は立方状～円柱上皮に被覆され，嚢胞上皮の胞体内に粘液を含有し，内腔面に気管支上皮様の線毛が散見されて，胃原発気管支嚢胞と診断した．
