Abstract
下部直腸癌の術前検査にて偶発的に発見され，癌手術に際して同時切除した大動脈分岐部に位置した後腹膜血管筋脂肪腫の1例を経験した．症例は85歳の女性，腹部CTにて腹部大動脈分岐部直上右側に，辺縁が強く造影される径4cm弱の分葉状腫瘤が見られた．MRIではT2強調像にて，やや不均一ながら明瞭な高信号を呈した．Paragangliomaや血管腫を術前診断として考えたが確定には至らず，直腸癌手術時に同時に切除した．肉眼的には黒色調の腫瘤であり，病理組織学には脂肪組織と海綿状組織に増生する小血管，平滑筋組織がみられた．免疫組織化学染色で，desmin陽性，HHF-35陽性，SMA陽性であり血管筋脂肪腫と診断した．血管筋脂肪腫は，主として腎臓に発生する比較的まれな良性の間葉系混合腫瘍であり，腎臓以外の部位に発生する血管筋脂肪腫は極めて少ないことから，文献的考察を加え報告する．
