Abstract
症例は, 69歳男性.26歳ごろより末端肥大症を発症し, 43歳時に下垂体腫瘍切除術を受けた.2003年左股関節脱臼に対して人工股関節置換術を施行された際, 胸部CTで頸部から上縦隔にかけての腫瘤影を指摘され, 当院を受診した.胸部CT, MRIでは, 甲状腺から大動脈弓まで連続する直径12×7cmの前縦隔腫瘍が甲状腺と連続しており, 縦隔内甲状腺腫が疑われた.初診時の血中成長ホルモン(GH)は13.9ng/mlと上昇し, 頭部MRIで下垂体腫瘍の再発を認め, 活動性の末端肥大症と考えられた.縦隔腫瘍切除・甲状腺右葉亜全摘術を胸骨正中切開を併用して施行した.永久病理検査結果は濾胞甲状腺腫であった.末端肥大症は, 甲状腺腫などの腫瘍性疾患の発生率が高いことが知られており, その事を念頭においた観察, 全身検索が必要と考えられた.
