057 Long Term Results of Peri-urethral Neurolysis for Premature Ejaculation at the Time of Inflatable Penile Prosthesis (IPP) Placement

Abstract

Premature ejaculation (PE) is the most common form of male sexual dysfunction. PE is often not addressed in patient with end stage erectile dysfunction (ED) undergoing inflatable penile prosthesis (IPP) placement. In our experience these patients continue to complain of PE after IPP placement We have previously reported on the anatomic distribution of the dorsal nerve of the penis. The dorsal nerves run from the ishiocavernosum to the coronal sulcus with fractionation of the nerves 2 cm proximal to the coronal sulcus. Typically there are 3-7 branches of the dorsal penile nerve that ultimately enter the corpora spongiosum bilaterally. The dorsal nerve of the penis is the main somatosensory nerve innervating the phallus and has been implicated in the development of PE. We report our Results of selective peri-urethral neurolysis (PUN) at the time of inflatable penile prosthesis (IPP) placement for treatment of PE. To outline PUN and show the long term Results of this novel procedure for the treatment of premature ejaculation in patients with end stage ED undergoing IPP placement.
