Abstract
目的：喉頭亜全摘出術 (supracricoid laryngectomy with cricohyoidoepiglottopexy : SCL-CHEP) 後の新声門の形態を，3次元画像を構築して解析した。対象と方法：喉頭亜全摘出術を施行した中で，multidetector-row CTを施行した17症例。1.25 mmスライスで撮影し，3次元画像構築ソフトを用いて輪状軟骨，披裂軟骨，術後喉頭内腔粘膜を描出した。結果：1）軟骨の描出；輪状軟骨，披裂軟骨は全例で良好に描出でき，筋突起や声帯突起などが同定できた。披裂軟骨の片側残存例も両側残存例も運動は良好で，脱臼所見はなかった。2）気道の描出；2種類に分けられ，発声時に気道が正中に一つの正中型と，披裂軟骨より外側に気道がみられる外側型があり，正中型の方がVHIは低かった。考察：新声門の形態は直接的に音声機能に関与する。従来では観察できなかった3次元的な形態の把握は，効率的な発声のために必要な要素を明らかにした。この検討を元に，より良い音声機能のために手術手技を改善することが必要である。
