Abstract
下肢血管内治療（Endovascular Therapy: EVT）を施行する際，特に慢性完全閉塞病変に対する治療手技には工夫を要する．ガイドワイヤーの選択やガイドワイヤー先端の形状は手技的に重要な要素である．また，ガイドワイヤーが閉塞血管内のどこを進んでいるかの情報は重要で，その一助になるのが血管内超音波検査（Intravascular ultrasound: IVUS）や体表面超音波検査である．特に，ワイヤーが閉塞部を通過する際の入口と出口の情報は特に重要であり，超音波検査を用いた手技は，血管内治療を安全に施行するためだけでなく，治療成功の鍵を握るといっても過言ではない．その中で，ステント内再狭窄病変や慢性閉塞病変または造影剤アレルギーの既往のある症例や造影剤の使用量を控えたい慢性腎臓病症例などにも，体表面エコーガイド下EVTが有用と思われる．今回，当院で行った実際の症例を提示し，その有用性と限界を論じたい．
