Abstract
症例は75歳，男性．腹痛を主訴に近医を受診し，イレウスの診断で紹介となった．CTにて腹腔内に多房性巨大嚢胞を認め，腹腔内臓器が右方に圧排され一部の小腸が拡張していた．巨大嚢胞性腫瘤によるイレウスと診断し，手術を施行した．腫瘤は後腹膜に存在し，腸管を腹側に圧排していた．腫瘍は左腎を巻き込んでいたため，これを合併切除した．切除標本は30×28cmの多房性嚢胞性腫瘤であった．病理組織学的検索にてリンパ管腫と診断した．後腹膜リンパ管腫は緩徐に増大することが多いとされるが，本症例のように比較的急速に増大する場合もあり，注意深い治療選択が必要と考える．
