Abstract
症例は大動脈炎症候群の診断を受けた34歳の女性.出産を強く希望しており,術後抗凝固療法を必要としないFreestyle生体弁(Freestyle弁)を用いて大動脈基部置換を施行した1例を報告する.精査で大動脈弁輪径30mm,Valsalva洞径43mm,大動脈弁閉鎖不全(Seller's III度),上行大動脈瘤(最大径50mm)を認めた.Freestyle弁(29mm)とHemashield人工血管(30mm)を用いて大動脈基部および上行大動脈を置換した.Valsalva洞周辺は炎症による癒着が強く,剥離困難であるため,冠動脈の再建はCabrol変法に準じて行った.術後経過は良好であった.大動脈炎症候群によるFreestyle弁に対する遠隔期の影響はまだ不明だが,術後のQOLを配慮し,Freestyle弁の応用は重要な選択肢の一つだと思われる.
