Abstract
Annuloaortic ectasia を伴うII型大動脈解離に Bentall 手術を行った後, 遠位側の残存偽腔に vegetation を伴う人工血管感染を合併したまれな症例を経験した. 症例は16歳, Marfan 症候群の男性. 15歳時に急性大動脈解離のため当院で Bentall 手術を施行した. 術後5カ月頃から発熱が出現し再入院した. 経食道心エコーでは, 人工血管末梢側の吻合部に近接してエントリーを認め, 偽腔は開存していた. また vegetation はエントリー部に付着していた. その後, 右下肢に急性動脈塞栓症を併発し, vegetation の一部が消失したので, 緊急手術を行った. 術中所見では, 内膜は大動脈末梢側吻合部の縫合線において一部脱落しており, vegetation は同部位に付着し偽腔内に充満していた. 弓部大動脈部分置換術, 大網充填, 右下肢の塞栓摘除を施行した. 術後20カ月目の現在, 発熱および炎症所見はなく, 社会復帰している.
