Abstract

Background Atrophy of prosthetic bed tissues increases over time for fully edentulous patients, thus creating unfavourable conditions for re-prosthetics. Objectives Improvement of treatment technique for patients with repeated complete laminar denture prosthetics of the mandible Methods 80 fully edentulous patients were examined, 50 of them underwent repeated prosthetics. Patients with repeated prosthesis were divided into two groups. The first group comprised 25 people, treated according to the standard method, while the other 25 patients were treated according to the improved method of local differentiated functional impression. In the process of examination, a study of compliance and functional load tolerance of the prosthetic bed mucosa and measurement of the tongue width in 3.3–4.3 tooth region was carried out, as well as estimation of ingestion and mastication function, saliva secretion speed and viscosity. Fixation and stabilization of new complete dentures were also evaluated. Results The results obtained confirm that patients who underwent repeated prosthetics with complete laminar dentures suffered from increased atrophy of the mandible alveolar part and prosthetic bed mucosa, and enlarged tongue. However, despite significant atrophy, prosthetic bed mucosa was found to be two times more load resistant as compared to the patients with primary prosthetics. Conclusions The data obtained allow the technique of local differentiated functional impression to be effectively applied, thus improving fixation and stabilization of complete dentures by 1.5 times. The application of the described method reduces adaptation time to new complete laminar dentures due to a faster recovery of mastication and swallowing functions as well as velocity and viscosity of saliva secretion.
