Abstract
肩関節脱臼骨折の中でも稀である上腕骨2 part外科頚骨折を伴った脱臼骨折に対し，K-wireを使って骨頭を経皮的に整復した後に，Fox tail nailを使って骨折部を展開することなく骨接合を行った症例を報告する．【症例】68歳 女性，2007年10月，自宅廊下で足を滑らせて転倒し受傷した．救急搬入後に静脈麻酔下での徒手整復を試みるも整復できなかったため，手術場で全身麻酔下にK-wireを経皮的に刺入して骨頭を整復した．受傷後5日目に外科頚骨折に対しFox tail nailを使った骨接合術を行った．術後1年，骨癒合，関節適合性は良好で特に障害なく日常生活を行っている．肩関節脱臼骨折において脱臼した骨頭が経皮的に整復できれば，その後の骨折に対する治療法の選択肢が広がり，より低侵襲で適切な治療を行うことができる．経皮的整復は脱臼骨折の型に構わず試みてよいのではないかと考えている．
