Abstract
埋込式骨延長器を使用して頭蓋拡張を行った1例を報告する.症例は3歳, 男児で, 誕生直後に後頭部脳髄膜瘤修復術と脳室-腹腔短絡術を受けた.徐々に舟状頭蓋変形が進行し, 後頭部の二分頭蓋による骨欠損部が著明に膨隆した.手術では, コの字型の骨切りにより両側頭頂後頭部に有茎の骨弁作成した.両側傍正中の骨縁に埋込式骨延長器を取りつけた.1日0.5〜1mmの骨延長を3週間施行し, 片側17.5mm, 両側35mmの拡張を行った.術後頭蓋骨横径が増大, 後頭部骨欠損の膨隆もなくなった.本法には, 急性期の硬膜外死腔や皮膚伸展が避けられる, 単純な骨切りで硬膜や骨膜の剥離が少ないので出血が減り, 骨弁への血流も保たれるなどの利点がある.今後, 適応や長期予後の検討, 器具の改良などが必要である.
