Abstract
本研究では選択的抗トロンビン作用を有するargatrobanのendothelin-1 (ET-1), thrombomodulin (TM) 動態のみならず凝固線溶系への影響について検討した.Argatroban投与群を9例, コントロール群の7例を対象とした.Argatrobanは第1, 第2病日には60mg/日を投与し, 第3より第7病日までは10mg/日を投与した.Argatroban投与群における血漿ET-1値は, 発症直後4.5±1.7pg/mlを示し, 第1病日には2.9±1.4pg/mlに減少し, コントロール群の第1病日の値と比較して, 有意に低かった (p<0.05).Argatroban投与前後において血中TMは有意な変動を認めなかった.Argatroban投与前後の血中TMは, age-match controlの血中TM値の13.9±2.3U/mlに比較して有意に高値を示した (p<0.05).Argatroban投与群のD-dimerの上昇は, コントロール群に比較し, その上昇の程度が低い傾向を示した.トロンビンの選択的阻害剤であるargatroban投与により, 血漿ET-1動態, TM動態, 血液凝血系への影響が本研究により明らかとなった.
