Влияние афлиберцепта на ХНВМ II типа

Abstract

The application of angiogenesis blockers is a main method of therapy for a wet form of age-r elated macular degeneration (AMD). Purpose. To determine an effect of aflibercept on morphological parameters of the retina and functional indicators of the macula in the formation of the type II neovascularization (CNV) in patients with agerelated macular degeneration. Material and methods. The study involved 28 patients with CNV type II associated with AMD. The average age of the patients was 62±5 years. Antivasoproliferative treatment with aflibercept was chosen as a therapy. The treatment was carried out according to the protocol ≪treat and extend≫ using LD. Before and after each intravitreal injection of the drug, a complete ophthalmological examination was performed. Results. A change in the light sensitivity was revealed in the zones 1–4 of the pattern by 4.89 dB, 5.51 dB, 1.62 dB, 5.67 dB compared with the data before treatment after the 1st injection, by 6,7 dB, 6.96 dB, 6.68 dB, 5.85 dB after the 2nd injection, by 9.89 dB, 7.5 dB, 7.42 dB, 7.24 dB, respectively, after the 3rd injection. The height of the neovascular membrane decreased after the 1st LD (decrease by 5.18 microns), after the 2nd LD – 37.69 microns (p<0.05 to the values before treatment), the 3rd – 39.42 microns (p<0.05 to the values before treatment). Similar dynamics determined a depression and a photoreceptor component as well: a decrease by 0.43 μm, 41.13 μm, 47.76 μm (p<0.05 to the values before treatment), respectively. Conclusion. The changes in the morphological and functional parameters of the retina using aflibercept were studied. The obvious positive dynamics of the anti-vasoproliferative drug demonstrates an efficiency of angiogenesis blockade in case of the neovascular membrane localization above the retinal pigment epithelium zone that determines an ability to impact on the macular area most effectively.
