Abstract

목적: 본 연구에서는 토릭소프트콘택트렌즈를 일정시간 착용했을 때와 응시방향을 달리하였을 때 각막이심률이 축 회전에 미치는 영향에 대해서 알아보고자 하였다. 방법: 이중쐐기형 축 안정화 디자인을 가진 토릭소프트콘택트렌즈를 85안의 직난시안에 착용시키고 착용 15분 후와 6시간 후에 각각 응시방향에 따른 회전방향 및 회전양 변화를 측정하고 각막이심률에 따른 차이가 나타나는 지 비교분석하였다. 결과: 토릭소프트콘택트렌즈의 회전은 착용 15분 후와 6시간 후 모든 각막이심률 군에서 귀-위쪽방향으로의 응시를 제외하고 모두 귀쪽으로 회전하는 경향이 있었다. 정면주시할 때의 회전양은 착용 15분 후와 6시간 후 모두 각막이심률이 증가할수록 회전양이 감소하는 음의 상관관계를 나타내었으며, 이심률이 작은 각막에서는 <TEX>$10^{\circ}$</TEX> 이상의 회전을 보이는 경우가 많았다. 토릭소프트콘택트렌즈 착용 15분 후의 회전양은 각막이심률에 따른 차이가 적었으나, 착용 6시간 후에는 각막이심률에 따라 회전양의 변화 정도에 차이가 나타났다. 결론: 본 연구에서는 토릭소프트콘택트렌즈 착용 시 각막이심률에 따라 축 회전양이 크게 달라지며 일정시간 착용 후의 회전양 또한 각막이심률에 영향을 받음을 밝혔다. 따라서 각막이심률에 따라 적절한 토릭소프트콘택트렌즈의 선택이 필요함을 제안한다. Purpose: The present study was aimed to investigate the effect of corneal eccentricity on the axial rotation when wearing toric soft contact lenses were worn for certain time and changing the gaze directions. Methods: Toric soft contact lenses with double thin zone design applied on 85 of with-the-rule astigmatic eyes. Then, rotational direction and amount of contact lenses were measured after 15 minutes and 6 hours of lens wear. The difference was further compared and analyzed according to corneal eccentricity. Results: The rotation of toric lens showed a tendency to rotate to temporal direction in all gaze directions except temporal-upper direction in all groups of corneal eccentricity. The amount of lens rotation in the frontal gaze direction exhibited a negative correlation since the amount was decreased with increasing corneal eccentricity after both 15 minutes and 6 hours of lens wearing. In many cases, the cornea with small eccentricity also showed the lens rotation larger than <TEX>$10^{\circ}$</TEX>. The difference in rotational amount after 15 minutes of toric lens wear was small according to the corneal eccentricity however, the change of rotational amount of contact lens according to corneal eccentricity was shown after 6 hours of lens wear. Conclusions: The present study revealed that the amount of axial rotation was largely varied according to the wearer's corneal eccentricity when wearing toric lens and the rotational amount after certain time of lens was also affected by corneal eccentricity. Thus, it is suggested that the selection of toric soft contact lenses based on corneal eccentricity is necessary.
