Abstract

Blepharoplasty is a surgical procedure aimed at creation of attractive aesthetic eyelids appearance with mandatory preservation of natural shape and position of the palpeblal fissure. Cicatrical eversion of the lower eyelid which is observed in 1 to 15 % of patients in cases with radical removal of skin, fat and muscles during this operation is the most severe complication after surgery. This paper presents a case of combined surgical correction of bilateral lower eyelids cicatrical eversion grade 3–4 in a 57 years old male suffering from diabetes mellitus after failed circular blepharoplasty. On the right eye, at first free skin grafting with an autograft taken from preauricular face zone was performed to replenish lack of tissues; then pentagonal resection of a lower eyelid portion and punctoplasty were carried out. On the left eye, one-step surgery including eradication of lower eyelid eversion using pentagonal resection method with free skin grafting was performed. The result of surgery was estimated in 3 and 6 months after the operation. Eradication of lower eyelids retraction and eversion, restoration of eyelids closing and cessation of epiphora were marked. The suggested technology allowed good functional and aesthetic result achievement, replenishment of tissues lack, epiphora cessation and elimination of severe complications risk.
