Abstract

The limited financial capabilities of regional healthcare require administrators to constantly search for new ways to optimize the costs of procurement of drugs, especially for resource-intensive nosologies, among which oncological diseases play a key role. The purpose of the work was to highlight the experience of implementing a system of centralized monitoring and procurement of antitumor drugs to provide regional hospitals, as well as to demonstrate the organizational effects of this management decision. The research materials included information on the procurement of antitumor drugs in 38 medical organizations for 106 drugs, presented in 156 reports on the newly approved form of monitoring their availability, including purchased drugs under 42 joint state contracts in 2023 and independently concluded contracts of medical organizations with suppliers outside the centralized system. The materials of the study were also local regulatory and administrative documents issued during the initiation and operation of the system of centralized purchase of chemotherapeutic drugs by the region in 2022-2023. Research methods: structural, system analysis, content analysis of regulatory documents and reporting documentation of medical organizations. As a result of the study, an algorithm was compiled for the implementation of a centralized procurement management by medical organizations within the Sverdlovsk region, indicating the necessary administrative steps towards the creation of this system, as well as the recommended form of reporting documentation for controlling the volume of procurement of drugs. The work showed the positive aspects of the regional experience and problematic aspects that require further solutions. This experience is recommended for implementation in other regions using the developed form of monitoring the availability of antitumor drugs in hospitals.
