The gender gap in stone-related surgery: A comprehensive analysis from an Israeli perspective.

To investigate the gender gap in the context of stone-related surgery within an Israeli population. We conducted a retrospective cohort study using administrative databases from Clalit Health Services to identify adults aged 18 and above who had their initial surgical treatment for upper tract urolithiasis. We employed descriptive statistics to outline the baseline patient characteristics, and the Cochran-Armitage test for trend was utilized to analyze surgical trends. Between 2003 and 2020, a total of 36,624 adult patients underwent surgical treatment for upper tract urinary stones. The mean age of patients was 55.01 years (standard deviation (SD) 16.6) for ureteroscopy (URS), 55.05 years (SD 15.1) for percutaneous nephrolithotripsy (PCNL), and 51.07 years (SD 15.1) for shockwave lithotripsy (SWL). When considering the distribution of procedures by gender, males accounted for 69.5% of URS cases, 58.3% of PCNL cases, and 70.6% of SWL cases, whereas females represented 30.5%, 41.7%, and 29.4% of URS, PCNL, and SWL cases, respectively. Across all surgical modalities, the male-to-female ratio exhibited fluctuations without a consistent trend, with both increases and decreases observed. In URS, the ratio saw a modest increase from 1.967 in 2003 to 2.173 in 2020. For PCNL, the ratio initially increased from 2.361 in 2003 to 2.549 in 2014, followed by subsequent fluctuations, but an overarching trend was not apparent. In contrast, for SWL, the ratio decreased from 2.15 in 2003 to 1.32 in 2020, with varying changes in between. This study highlights the dynamic nature of gender gap in stone-related surgery outcomes. While the male-to-female ratio exhibited fluctuations over a 17-year period, no consistent trend emerged. The absence of a clear trend underscores the complex and multifaceted factors influencing the gender gap in urolithiasis.

Methods for Assessing Renal Нemodynamics after Minimally lnvasive Treatment of Urolithiasis

The purpose of research. Urolithiasis occupies a leading place among all urological diseases after infectious and inflammatory ones. /n addition, it does not lose its relevance due to the fact that the number of patients with this disease is steadily increasing. Тhe aim of the study is to study the peculiarities of changes in renal hemodynamics after minimally invasive methods of treatment of urolithiasis remote shock wave lithotripsy (DLТ) and percutaneous nephrolithotripsy (РNL) to improve the quality of medical care for urological patients.Methods. Numerous studies have shown in detail the effectiveness of using remote shock wave lithotripsy and percutaneous nephrolithotorypsy in the treatment of kidney stones. Тhese methods are minimally invasive, well tolerated by patients, and reduce the risks of complications, but they are accompanied by kidney injury, in particular, microcirculation disorders. Тhe use of the Dopplerographic method of investigation allows for a highly informative assessment of the degree of impaired renal microcirculation for the diagnosis of urolithiasis, including for the assessment of hemodynamics after minimally invasive treatment methods.Results. Тhe work shows that when using dopplerography of renal vessels after remote shock wave lithotripsy and percutaneous nephrolithotorypsy, a violation of hemodynamics after DLТ is noted in dynamics already on the first day. In addition, the terms of normalization of indicators are longer in comparison with hemodynamics after РNL. Аlso, with РNL, there is a gradual increase in blood flow indicators in the dynamics of the postoperative period.Conclusion. Тhe work shows that ultrasound Dopplerography is an effective method of dynamic assessment of renal blood flow, and the indicator of renal damage is the resistance index, mainly in the interlobular arteries.

Comparison of temperature and renal tissue thermal damage by holmium laser with different energy parameters during lithotripsy: in vitro porcine kidney model.

Holmium laser percutaneous nephrolithotripsy was simulated by porcine kidney calculus model in vitro to investigate thermal damage of renal tissue by different energy parameters of the holmium laser. We placed human kidney calculus specimen in fresh vitro porcine kidney, then insert thermocouple temperature probes into the submucosa of the renal pelvis and reheated in a 37°C water bath. A percutaneous nephrological sheath was used to penetrate the renal parenchyma with a moderate irrigation rate of 30ml/min at 18℃. The Holmium laser was used to fragment the stones under a nephroscope, and the temperature was recorded. The four independent models were lithotripsy with 30W and 60W laser for 5 and 10min, respectively; the mean temperature of 30W vs. 60W within 5min was 36.06°C vs. 39.21°C (t = 5.36, P < 0.01) and the highest temperature was 43.60°C vs. 46.60°C; the mean temperature of 30W vs. 60W within 10min was 37.91°C vs. 40.13℃ (t = 5.28, P < 0.01), maximum temperature 46.80℃ vs. 49.20℃. Pathologically, each kidney was observed to have different degrees of thermal damage lesions, and the higher power and longer time the more severe the injury, but the injury was mainly limited to the uroepithelial and subepithelial tissues, with rare damage to renal tubules. The higher laser excitation power and longer duration raised the intrarenal temperature significantly and caused a certain degree of thermal damage to the kidney tissue, but overall it was found to be safe and reliable. Urologists can avoid further side effects through surgical expertise.

“Forgotten” Encrusted Ureteral Stents, Combined Endourological Approach

Introduction: Ureteral stents have been widely used for drainage of the upper urinary tract during both emergency and elective surgical procedures since 1967. The main pathology in which these stents are used is urolithiasis.Objective: To present our experience with the surgical treatment of patients with “forgotten” encrusted ureteral stents using a combined endourological approach.Materials and methods: Patients with encrusted ureteral stents who underwent endourological procedures from 2016 to 2022 were prospectively evaluated. They were grouped based on the degree of stent encrustation according to the FECal classification. The duration of stent placement, number and types of surgical procedures, number of procedures before complete removal of the stent and concrements, surgery duration, hospital stay, complications, concrement analysis, and frequency of complete concrement removal per surgery were compared between the groups. The combined endourological procedure was performed with the patient placed in the Galdakao-modified supine Valdivia position. This position allows simultaneous antegrade and retrograde endourological access.Results: The study included 46 patients grouped according to the FECal classification. In 38 patients, stents were successfully removed in a single procedure. The mean operative time, concrement-free status, and complication rate were 90.2 ± 19.8 minutes, 78.3%, and 32.6%, respectively. Total encrustation volume was higher for grades IV and V (5.6 ± 1.8 and 7.6 ± 2 cm3) compared with all the other grades. Percutaneous nephrolithotripsy and cystolithotripsy were the most common procedures in grades IV and V. Lithotripsy was commonly used for ureteral stent encrustation, especially in the groups with grades I and III. The operative time was longer in groups IV-V compared with groups I and II because percutaneous nephrolithotripsy was more frequent in these cases. Retrograde intrarenal surgery was performed in 88% of the cases with the combined approach to check whether concrements were present in the renal calyces and pelvis. In 5 cases, a flexible ureterorenoscope was advanced antegradely through a percutaneous access to disintegrate the encrustation of the proximal part of the stent’s ureteral section. Complications occurred in 32.6% of the patients. Most complications (26%) were minor: fever, pain, or gross hematuria. One case required segmental renal artery embolization for bleeding, and antibiotic therapy associated with an attack of pyelonephritis was adjusted in 2 cases.Conclusions: The endoscopic combined approach in the Galdakao-modified supine Valdivia position is a safe and effective technique that allows removal of “forgotten” encrusted stents, in most cases, in a single procedure. The FECal classification seems to be useful for surgical planning and prognosis.

Learning curve of ultrasound-guided percutaneous nephrolithotripsy in the treatment of complex renal calculi.

This study was performed to investigate the learning curve of ultrasound-guided percutaneous nephrolithotripsy (PCNL) for the treatment of complex renal calculi. Seventy-two patients with complex renal stones who underwent ultrasound-guided PCNL by the same operator from November 2016 to September 2020 were retrospectively analyzed. The learning curve of PCNL for complex renal stones was analyzed using multifactorial cumulative sum (CUSUM) analysis and best-fit curves. The CUSUM best-fit curve peaked at Case 36, which represented the minimum number of cumulative cases required to cross the learning curve for this surgical procedure. Accordingly, the learning curve was divided into a learning improvement period (36 cases) and a proficiency phase (36 cases). Compared with the learning improvement period, the proficiency phase was characterized by a shorter puncture time and operation time, a smaller drop in the hemoglobin level, and a lower postoperative complication rate. The learning curve of PCNL for the treatment of complex renal calculi can be divided into a learning improvement period and a proficiency phase, and the minimum cumulative number of cases is 36.

Advantages and disadvantages of percutaneous surgery of urolithiasis in the patient’s supine position

BACKGROUND: Studying the clinical features of different surgical approaches in patients with urolithiasis with stones localized in the kidney and the upper third of the ureter is an important practical issue in modern urology.&#x0D; AIM: The aim of the study is a clinical evaluation of the results of percutaneous surgery for urolithiasis with the patient in the supine position.&#x0D; MATERIALS AND METHODS: The results of surgical treatment of 316 patients with urolithiasis with stones localized in the kidney (more than 10 mm) and the upper third of the ureter (more than 8 mm) are presented. All patients underwent percutaneous nephrolithotripsy in the supine position.&#x0D; RESULTS: Complete elimination of stones was achieved in 91.3% of patients with stones of the upper third of the ureter and 96.2% of patients with kidney stones. Clinical evaluation of the surgical results showed that the frequency of intraoperative complications of traumatic origin is about 10%, the most common complication being bleeding. At the same time, complications of grades IV and V according to the Clavien–Dindo classification were noted. Performing percutaneous nephrolithotripsy in the supine position reduces the time required to position the patient, allows for the combination of transurethral and percutaneous interventions, is more effective for patients with increased weight and concomitant diseases, and also provides a significant advantage in the need for ventilation and possible resuscitation.&#x0D; CONCLUSIONS: It is advisable to introduce a personalized approach to the choice of surgical access when performing invasive interventions on the upper urinary tract.

A NOVEL TECHNIQUE SINGLE SITE MULTIPUNCTURES SUPINE PCNL: CASE SERIES REPORT

Objective: To presents a case series of three patients who received PCNL during 2020 and 2021. Case(s) Presentation: We reported three cases of two females and one male, aged 54, 63, and 48. All patients were diagnosed for having the staghorn stones in the kidney. We performed the “Single Site Multipuncture Supine (SMS) PCNL” technique on these patients. Discussion: Percutaneous Nephrolithotripsy (PCNL) is the preferred surgical technique for treating staghorn or other complicated kidney stones. There are many techniques and modified positions available for PCNL. This technique removes complex kidney stones by making a one cm skin incision to accommodate multiple punctures to access all kidney poles. The highest reduction in hemoglobin was only 1.54 g/dL in the patient who got four punctures, no blood transfusion was needed during post-operative recovery, and no complications occurred during the follow-up period. All patients were stone-free post-operatively without any need for additional procedures. The mean time of overall surgery was 130 minutes. Conclusion: This single-site multi-punctures supine PCNL technique could give the advantage to cosmetics due to less tissue injury.&#x0D; Keywords: Multi punctures, PCNL, staghorn stones.

Extracorporeal shock wave lithotripsy, ureterolithotripsy, and percutaneous nephrolithotripsy challenges in managing spinal cord neuropathy patients. Lessons learned from a scoping review.

We aim to review the outcomes of shock wave lithotripsy (SWL), ureteroscopy, and percutaneous nephrolithotripsy (PCNL) for renal and ureteral stones in spinal cord neuropathy patients (SNP). A literature search was performed on 8th March 2023 using PubMed, EMBASE, and Google Scholar with no date limit. Preclinical/animal studies, reviews, letters to the editor, case reports, and meeting abstracts were excluded. Only English papers were accepted. Thirty-five articles were accepted. Five studies focused on SWL, 17 on PCNL, and 6 on ureteroscopy. The remaining articles employed more than one procedure. Stone composition has shifted from struvite to the more common calcium phosphate. SWL showed a very poor stone-free rate (SFR) likely due to challenges in patient positioning, stone visualization, localization, and inability to pass fragments spontaneously. Flexible ureteroscopy and PCNL were associated with a high incidence of infectious complications, long hospital stays, high blood transfusion rate, and intensive care admissions. There were also cases of death. Both procedures were challenging due to genitourinary reconstruction, scoliosis and kyphosis, rib-cage deformity, lower limb contractures, and severe comorbidity which also affected anesthesia. SFR was lower than in non-neurological patients. SWL, ureterolithotripsy, and PCNL should be considered challenging procedures in SNP due to positioning issues, an increased risk of intra and peri-operative morbidity, and even mortality. Computed tomography should be recommended to assess residual fragments as it becomes imperative to minimize a re-intervention in SNP who should be preferably treated in referral centers.

Renal Phaeohyphomycosis: Aureobasidium Pullulans Infection Caused by Instrument Inoculation in a Healthy Patient - A Case Report

Introduction: Aureobasidium pullulans, ablack yeast-like dematiaceous fungus. Most ofthese infections commonly affect the immunocompromised patients; however, post-instrument inoculation was extremely rare. This is the first report of renal phaeohyphomycosis by A. pullulans after stone surgeries reported worldwide, despite a thorough literature review. Case Report: A 29-year-old immunocompetent native lady, with underlying bilateral grade II renal parenchymal disease, was referred for right staghorn calculi and big left ureteric stone with bilateral hydronephrosis back in 2016. She underwent right percutaneous nephrolithotripsy (PCNL) and left ureteroscopic surgery (URS). Intra-operatively, 90% stone clearance for the right kidney and complete stone clearance was achieved over the left ureteric stone. Blood investigation showed Urea was 8.1 and Creatinine was 200 respectively. There was no growth in Urine C+S sent intraoperatively. Post-operatively, she did a DTPA scan which showed moderate impaired right kidney function (28ml/min-69%) with a non-functioning left kidney (13ml/min-13%). Her condition and renal function had been closely monitored during follow-up in the urology clinic. During follow-up, computed tomography urography (CTU) reassessment in 2018 showed residual right renal stones and resolved left hydronephrosis. Subsequently, she underwent right retrograde intrarenal surgery (RIRS) and ureteric stenting. Intraoperatively, complete stone clearance for the right upper and middle pole; there were a few blackish tubular foreign bodies seen at the right lower pole which were extracted and sent for investigations. Interestingly, the histopathology (HPE) reported as blood clots while the fungal PCR done by the Institute for Medical Research (IMR) showed Aureobasidium pullulans DNA detected. Post-operatively, she completed a one-month course of anti-fungal antibiotic and was discharged home well. Subsequent urine culture was cleared and CTU assessment showed no evidence of stone recurrence or foreign bodies. Discussion and Conclusion: Renal phaeohyphomycosis caused by Aureobadium pollulans secondary to post-instrument inoculation is first reported worldwide in an immunocompetent native lady.

A retrospective cohort study, analyzing trends in management of upper urinary tract stones in the adult Israeli population.

To assess trends of surgical intervention in adults with upper urinary tract stones in Israel. A retrospective cohort study utilizing administrative databases held at Clalit Health Services, to identify all adults (⩾18 years) who underwent their first surgical treatment for upper tract urolithiasis. Descriptive statistics were employed to summarize baseline patient demographics and surgical trends were analyzed using the Cochrane-Armitage test for trend. Between 2003 and 2020, 36,624 adult patients were treated surgically for upper tract urinary stones. Mean age was 53.6 years (SD16.1). During the period investigated, the number of insured by Clalit Health Services increased by 25% and the total number of surgically treated stones increased by 98.7%. By type of procedure: Ureteroscopy (URS) increased by 351%, percutaneous nephrolithotripsy (PCNL) increased by 67%, shockwave lithotripsy (SWL) declined by 79%. The number of procedures per 100,000 population grew from to 37.5 in 2003 to 58.05 in 2022. The percentage increase in total number of surgical procedures was 103% and 90% in males and females, respectively. Our findings reveal significant increases in the total number of surgically treated stones over the investigated period. Notably, this increase far outpaced the growth in the number of individuals insured by Clalit Health Services. Further research and interventions are warranted to explore the underlying factors driving these trends and to develop targeted approaches for prevention, early detection, and minimally invasive treatment of upper urinary tract stones in Israel.

SAFETY AND EFFICACY OF SEMI-RIGID URETEROSCOPE IN RETROGRADE INTRARENAL SURGERY FOR RENAL PELVIC STONES UPTO 2 CM

Objectives: To determine the efficacy and safety of RIRS with rigid URS for renal pelvis stone  between 1 to 2cm. Materials and Methods: This is a prospective observational case series of 48 patients who under  RIRS at Institute of Kidney Diseases, Peshawar during 1st January 2022 and 31st December The primary indication was single renal pelvic calculi of up to 2 cm diameter in patients with previously failed treatment modalities or as primary cases. Data was analysed regarding patient demographics, stone size, surgery time, complications and clearance rates within a 2 months follow-up period. Results: Total of Forty-eight patients with 29 (60.4%) males and 19 (39.6%) females in a ratio of 1.5 to 1. The mean age was 38.7 ± 10.8 years (range: 19 – 60 years). Mean stone diameter was 15.3 ± 2.6 mm. Early postoperative complications included fever, haematuria and retained stones. There were 12 (25%) cases of postoperative fever who resolved without any major sequelae, and  11 (22.9%) cases of early postoperative haematuria of which 4 (8.3%) patients received blood transfusions. The overall clearance rate after fi rst RIRS was 87.5% (n = 42) with a P value of 0.01 and is clinically signifi cant. Six (12.5%) patients were found to have stone fragments retained.  The clearance rate after 2nd look RIRS was 95.83% (n = 46). Conclusion: Retrograde intrarenal surgery using semi-rigid ureteroscope for renal pelvic stones  has favourable stone free rates and the results are comparable to percutaneous nephrolithotripsy. Complications requiring intervention are below ten percent and are non-fatal.

Introduction. Currently, percutaneous nephrolithotripsy is the main treatment for large and coral-like kidney stones in children. The purpose of the study was to increase the effectiveness of percutaneous nephrolithotripsy (PCNL) in children using miniature-sized instruments. Materials and methods. For the period from 2008 to 2019, 320 children aged 1 to 17 years underwent nephrolithotripsy in the pediatric department of the N. Lopatkin Scientific Research Institute of Urology. Depending on the size of the instruments used, patients were divided into 3 groups. The first group included children who underwent surgery using the Mini-PCNL technique (n-211), the second group included patients operated on using the Ultra-Mini-PCNL method (n-16) and the third group included children whose percutaneous intervention was performed using a standard-sized nephroscope (n-93). The intervention was carried out according to the standard method; all patients underwent cystoscopy, catheterization of the ureter, access formation, lithotripsy, lithoextraction and installation of nephrostomy drainage. Results. In the first group of children studied, complete elimination of clinically significant stones (residual stones smaller than 4 mm) after the initial intervention was achieved in 169 (80.1%) pediatric patients. In the second group, there was 13 (81.2%) children. And in the third group, getting rid of stones was achieved in 73 (78.5%) patients. The average time required to perform surgery in all three study groups was comparable and amounted to 45 (40-125) minutes in the first group, 40 (25-85) minutes in the second, and 50 (40-180) minutes in the third group. The final efficacy of percutaneous nephrolithotripsy in children using instruments of various sizes in the first group was 89.6%, in the second – 93.7% and 88.1% in the third study group. The hospital stay after surgical treatment was: 4 (3-12) days for the first group, 3.5 (3-7) and 4.5 (3-17) days for the second and third groups, respectively. The need for additional interventions after PCNL in pediatric patients was in 69 (32.7%) children in the first group, 4 (25%) in the second and 41 (44.1%) in the third group. Conclusion. The use of mini-PCNL, ultra-mini-PCNL can reduce the duration of surgical intervention by 10% and 20%, the hospital stay after PCNL by 11.2% and 22.3%, the need for additional interventions by 11.4% and 19.1%. Can reduce the total number of complications by 10% and 12.2%, respectively, compared with the use of standard-size instruments. The use of Mini-PCNL in children is indicated in the younger and middle age groups, Ultra-Mini-PCNL is indicated for single stones up to 2 cm or as an additional access in the multi-accesses formation.

Introduction. Despite the development of minimally invasive technological methods for the treatment of urolithiasis, improving the functional state of the kidneys in the postoperative period does not lose its importance at the present time. The decrease and loss of renal function after surgical treatment of complex forms of nephrolithiasis in various ways creates the problem of choosing one or another surgical technique in order to avoid negative results of surgical treatment. modern urology. Purpose of the work: to assess the correlation between the volume of intraoperative blood loss and dynamic changes in glomerular filtration rate (GFR) in the postoperative period with various methods of surgical treatment of large and complex kidney stones. Purpose: to assess the correlation between the volume of intraoperative blood loss and dynamic changes in the GFR in the postoperative period with various methods of surgical treatment of large and complex kidney stones. Materials and methods. The study selected data from 97 patients with urolithiasis, large and complex kidney stones of the urological department of the city hospital No 2 of the city of Nur-Sultan of the Republic of Kazakhstan. The median age was 52 (23–73) years. The average area of a kidney stone was 428.3±36.1 mm2, its average density according to Hounsfield was 1042.9±34.8 HU. All patients were divided into 3 cohort comparison groups according to the method of surgical treatment: percutaneous nephrolithotripsy (PCNL) on prone position (1 group) – 44 patients (45.4%), PCNL on the supine position (2 group) – 29 patients (29.9%), laparoscopic pyelolithotomy (3 group)– 24 patients (24.7%). The median baseline GFR in the 1 group was 91 ml/min/1.73 m2, in the 2 group 95 ml/min/1.73 m2, in the 3 group 101 ml/min/1.73 m2. The average duration of surgical intervention for 1 group was 92.8±6.8 min, for 2 group – 119.6±10.9 min, for 3 group-146.9 ± 6.1 min. The mean blood loss in 1 group was 144.5±9.1 ml, in 2 group 235.3±19.1 ml, in 3 group 43.1±5.2 ml. Results. A decrease in GFR below 60 (ml/min/1.73 m2), corresponding to stage III of chronic kidney disease (CKD), was most pronounced in the 2 group: from 3 (3.1%) cases before surgery to 7 (7.2%) cases in postoperative period. A decrease in GFR below 30 (ml/min/1.73 m2), which corresponds to stage IV CKD, also had negative dynamics in the group of patients with 2 group: from 2 (2.1%) to 3 (3.1%) cases respectively. In the rest of the studied groups, there was no negative dynamics of GFR. A correlation was found between GFR in the postoperative period and intraoperative blood loss (r = 0.410, p&lt;0.001). Conclusions. Surgical treatment of urolithiasis of the kidneys is associated with risks of intraoperative blood loss, the volume of which directly correlates with the deterioration of the functional state of the kidneys in the postoperative period. Reducing the volume of intraoperative blood loss in various surgical methods for the treatment of urolithiasis helps to minimize the risk of developing acute kidney injury in the postoperative period.

Calcutta position: A new modified supine decubitus for supine PCNL.

PCNL (percutaneous nephrolithotripsy) is considered the gold standard treatment for renal stone more than 2 cm. In today's scenario, supine PCNL is considered equally effective as prone PCNL. The ideal position for supine PCNL is still debatable. We hereby describe our initial experiences of supine PCNL in a novel position. This prospective study includes 60 patients who underwent supine PCNL in the 'Calcutta position' in our institute from August 2021 to August 2022. Successful procedure was defined as a complete stone free rate or a clinically insignificant residual stone (<4 mm). Average Operative room (OR) occupancy time was 130.9 ± 19.63 min. The immediate stone free rate was 84.2%, 71.4% and 37.5% for single, multiple and staghorn calculus respectively. Complications include fever, requirement of blood transfusions and renal colic. The average hospital stay was 83.6 ± 17.42 h. Eight patients (13.3%) required secondary procedures like extracorporeal shock wave lithotripsy (ESWL) or relook PCNL. At 3 months average stone free rates were 92%, 85%, 75% for single, multiple and staghorn calculus respectively. We performed supine PCNL in Calcutta position in obese, kyphoscoliosis, poliomyelitis, autosomal polycystic kidney disease (ADPKD), malrotated kidney and diverticular stone with comparable success. Supine PCNL in Calcutta position is a safe and effective option for nephrolithiasis management. Apart from the inherent advantages of supine PCNL it also has the advantages of better C-Arm and nephroscope manoeuvrability. Supine PCNL in Calcutta position was performed in a variety of scenarios with comparable results.

Dynamics of inflammatory reaction markers in patients under endourological interventions

Introduction. Urolithiasis is one of the most widespread urological diseases and infectious complications remain one of the most common adverse events after surgical interventions for urolithiasis. Currently, in routine clinical practice in most clinics in the country, for the purpose of postoperative control and detection of infectious complications is used: a clinical blood test with assessment of the dynamics of the blood leukocyte count. Unfortunately, the current edition of the clinical guidelines does not contain recommendations on what indicators of postoperative follow-up examinations the clinician should focus on in order to identify infectious and inflammatory complications. For this reason, the study of the value of other markers of the inflammatory response appears to be an actual task.Objective. To assess the relationship between the dynamics of laboratory markers of infectious-inflammatory complications and the incidence of infectious-inflammatory response after endoscopic stone removal.Materials &amp; methods. The prospective cohort study included 217 patients diagnosed with urolithiasis and underwent surgical removal of stones by endoscopic method. Of total patient numbers, 43 patients underwent ureteroscopy (URS) with lithotripsy, 152 patients underwent percutaneous nephrolithotripsy (PNL), and 22 patients underwent flexible ureterolithotripsy with lithotripsy. An analysis of demographics, clinical parameters and postoperative complications was performed as a descriptive analysis. Patients underwent a standard list of laboratory and instrumental examination methods. The mandatory control of body temperature was carried out t.i.d. daily. Before surgical treatment, indicators of WBC, LYMP, CRP, and ESR content were recorded on the first and second days after surgical treatment.Results. Postoperative fever was recorded in 40 (18.5%) patients. During the postoperative period, an increase in CRP and ESR was observed in both the fever and the normal temperature group, exceeding the reference values on the first day. At the same time, in the fever group (in 40 patients — 18.5%), a significantly higher increase in CRP and ESR was observed (685% vs 323%) for CRP and (146% vs 80%) for ESR. According to the results obtained in our study, CRP and ESR indicators demonstrate the most clinically significant dynamics in patients with postoperative fever, increasing by more than 500% and 100%, respectively.Conclusions. Endoscopic removal of urinary stones is a safe treatment option in patients with sterile urine cultures. After endoscopic stone removal, all patients experience such reactive changes in peripheral blood parameters as: an increase in the level of leukocytes, lymphopenia, an increase in ESR and CRP levels. Indicators of CRP and ESR demonstrate the most clinically indicative dynamics.

Prevention of postoperative pyelonephritis in patients after percutaneous nephrolithotripsy for large and coral kidney stones

Background. Improving the results of prevention and treatment of postoperative pyelonephritis, the most frequent complication of percutaneous nephrolithotripsy, is an important task of modern urology.&#x0D; Aim. To determine the effectiveness of using physiotherapeutic procedures (magnetotherapy and laser therapy) for prevention of pyelonephritis development in the postoperative period.&#x0D; Materials and methods. The study included 72 patients with kidney stones of large size who underwent percutaneous nephrolithotripsy. The patients were divided into two groups: group 1 – 44 patients with kidney stone over 2.5 cm (including coral), group 2 – 28 patients with kidney stone over 2.5 cm (including coral) who underwent physiotherapeutic procedures in the perioperative period in combination with antibiotic therapy. All patients underwent percutaneous nephrolithotripsy with preoperative administration of antibacterial therapy. All patients underwent urine and stone culture, as well as laboratory and clinical evaluation of pyelonephritis attack development in the postoperative period.&#x0D; Results. Analysis of histories showed that in group 1, clinical symptoms of pyelonephritis exacerbation were observed more frequently in operated patients than in patients who underwent physical therapy in the perioperative period. The severity and duration of hyperthermia in the group 2 was noted to be not so high and prolonged. The percentage of microflora detection in urine and stone cultures was insignificantly different in both groups. At the same time, the level of leukocytosis in the group 2 in the postoperative period was on average lower, a 3-fold decrease in the frequency of prescribing "reserve" antibacterial drugs was also revealed.&#x0D; Conclusion. In patients with large and coral kidney stones physiotherapeutic procedures can be used in combination with antibacterial therapy for prevention and treatment of pyelonephritis exacerbation in the postoperative period after percutaneous nephrolithotripsy.

Применение «безнефростомной» перкутанной нефролитотрипсии у детей.

Introduction. Currently, percutaneous renal surgery is a common surgical procedure in urologic practice for the treatment of large and staghorn renal stones. Placement of a nephrostomy tube has been well recognized as a standard procedure to provide hemostasis, adequate drainage, and access for additional endoscopic procedures. At the same time, the use of a nephrostomy tube has negative consequences such as discomfort/pain symptom, longer hospital stay and possible urine extravasation. In view of the above, draining of the access tract raises questions in some cases. The aim of the study was effectiveness of tubeless percutaneous nephrolithotripsy in the treatment of urolithiasis in children. Materials and methods. Between 2013 and 2019, 66 patients (aged 1-17 y/o) underwent tubeless percutaneous nephrolithotomy (PNL) at the Department of Pediatric Urology of N. Lopatkin Scientific Research Institute of Urology and Interventional Radiology. Patients were divided into three groups depending on the size of the instruments and the way the operation was completed. The first group (n=28) underwent surgery using the mini-PNL technique with the closure of the access tract. The second group (n=21) underwent surgery using the mini-PNL technique in the end of the surgery, moderate bleeding along the tract was detected in order to stop bleeding, a hemostatic matrix with thrombin was introduced. The third group (n=17) underwent surgery using the standard PNL technique and moderate bleeding was also determined in this group with the aim of tubeless completion, a hemostatic matrix with thrombin was introduced. Drainage of the upper urinary tract in the postoperative period was carried out by "JJ"-internal and "J"-external ureteral stents, which were held for up to 14 days. Results. The complete removal of the calculus in one surgical session was similar in all three groups. In the first group there were 26 (92.8%) children, in the second group 19 (90.4%) and in the third group 16 patients (94.1%). The need for additional interventions after surgical treatment was 21.4% in the first group, 23.7% in the second and 17.5% in the third group. Febrile fever (persistent temperature 380 C) most often occurred in the first group – in 10.7% of cases, while in the second and third groups this complication was detected in 4.7% and 5.8% of cases, respectively. Gross hematuria in the postoperative period was noted in the group without the use of a hemostatic matrix, averaging 35 (2-115) hours. Conclusion. The proposed tubeless PNL with the use of a hemostatic matrix is indicated for children without bacteriuria, with complete removal of the calculus, in the absence of trauma of the pelvicalyceal system and intensive bleeding. The duration of gross hematuria decreased with the introduction of the matrix in standard tubeless PNL by 51.4% and in mini-PNL by 25.8% compared with the 1-st group

Planning the approach in mini percutaneous nephrolithotripsy according to individual kidney anatomy detected by computed tomography

Aim to analyze postoperative complications after mini percutaneous nephrolithotripsy, taking into account an assessment of individual variations of kidney anatomy.&#x0D; Material and methods. Postoperative complications after mini percutaneous nephrolithotripsy were analyzed in 60 patients who underwent preoperative CT urography with intravenous bolus contrast and 3D reconstruction.&#x0D; Results. We found no significant correlation between the number of complications after mini percutaneous nephrolithotripsy and the type of division of the branches of the renal artery of the 2nd order, as well as no dependence of the occurrence of complications on the presence of accessory arteries. A moderate correlation between the number of complications and the number of accesses was noted.&#x0D; Conclusion. In mini percutaneous nephrolithotripsy, the preoperative planning and marking of the access trajectory can significantly reduce the number of postoperative complications.

Efficacy of extracorporeal shock wave lithotripsy for post-transplant kidney stones. Hope or Disappointment?

Nephrolithiasis in a transplanted kidney is an important medical and social problem. The presence of renal calculi may not manifest clinically for a long time due to the peculiarities of the surgical intervention during organ transplantation. Development of chronic urinary tract infection and deterioration of the functional ability of the renal transplant in the presence of kidney stones can lead to graft death, which is an immediate threat to the patient’s life. Existing Russian guidelines on the treatment of urolithiasis currently lack a clear strategy for the management of kidney transplant recipients.Objective: to systematize literature data on analysis of the outcomes of extracorporeal shock wave lithotripsy (ESWL) and other methods in patients with post-transplant kidney stones.Results. Thirty-five publications on the research topic were selected. We summarized the information on various therapy options for patients with stones in transplanted kidney: endourological approach, ESWL, percutaneous nephrolithotripsy (PCNL), open surgical treatment (nephrostomy, pyelolithotomy). A modern foreign algorithm for the management of patients with post-transplant kidney stones depending on the severity of obstruction with sepsis and the size of the renal calculi is presented.Conclusion. 1. The presence of stones in a kidney graft is a clinical situation that requires surgical treatment. 2. In clinical practice, different methods of treatment can be used, such as open intervention, ESWL, PCNL, retrograde transurethral manipulations. 3. In most cases, patient management tactics depend on the clinical picture (presence/absence of obstruction) and the size of the calculi. 4. The use of ESWL, as the most frequently used method, testifies to its efficiency and low-traumatic effect.

Concerns about stone free rate and procedure events of percutaneous nephrolithotripsy (PCNL) for 2–4 cm kidney stones by standard-PCNL vs mini-PCNL- comparative randomised study

BackgroundTo compare the efficacy and safety of standard percutaneous nephrolithotomy (PCNL) with mini- PCNL for kidney stones 2–4 cm.MethodsEighty patients were enrolled in a comparative study, they were randomly divided into mini-PCNL group (n = 40) and standard-PCNL (n = 40). Demographic characteristics, perioperative events, complications, stone free rate (SFR) were reported.ResultsBoth groups showed no significant difference in clinical data about age, stone location, back pressure changes, and body mass index. The mean operative time was (95 ± 17.9 min) in mini-PCNL, and (72.1 ± 14.9 min). Stone free rate were 80% and 85% in mini-PCNL and standard-PCNL respectively. Intra-operative complications, post-operative need for analgesia, hospital stay were significantly higher in standard-PCNL compared to mini-PCNL (85% vs. 80%). The study followed CONSORT 2010 guidelines for reporting parallel group randomization.ConclusionMini-PCNL is an effective and safe treatment of kidney stones 2–4 cm, it has the advantage over standard-PCNL being has less intra-operative events, less post-operative analgesia, shorter hospital stay, while operative time and stone free rate are comparable when considering multiplicity, hardness, and site of stones.
