Airborne Allergic Contact Dermatitis: Management and Responsible Allergens on the American Contact Dermatitis Society Core Series.

Airborne allergic contact dermatitis occurs when allergen particles suspended in the air deposit on the skin. Many allergens can result in an airborne distribution if presented in an aerosolized form. This review summarizes the allergens in the American Contact Dermatitis Society Core Allergen Series that have been documented to cause airborne allergic contact dermatitis. In addition to responsible allergens, this article also reviews management and treatment options.

Airborne allergic contact dermatitis caused by isothiazolinones in water-based paints: a retrospective study of 44 cases.

Airborne allergic contact dermatitis caused by paints containing isothiazolinones has been recognized as a health hazard. To collect epidemiological, clinical and patch test data on airborne allergic contact dermatitis caused by isothiazolinone-containing paints in France and Belgium. A descriptive, retrospective study was initiated by the Dermatology and Allergy Group of the French Society of Dermatology, including methylchloroisothiazolinone (MCI)/methylisothiazolinone (MI)- and/or MI-sensitized patients who developed airborne allergic contact dermatitis following exposure to isothiazolinone-containing paint. Forty-four cases were identified, with mostly non-occupational exposure (79.5%). Of the patients, 22.5% of also had mucosal symptoms. In several cases, the dermatitis required systemic corticosteroids (27.3%), hospitalization (9.1%), and/or sick leave (20.5%). A median delay of 5.5 weeks was necessary to enable patients to enter a freshly painted room without a flare-up of their dermatitis. Approximately one-fifth of the patients knew that they were allergic to MI and/or MCI/MI before the exposure to paints occurred. Our series confirms that airborne allergic contact dermatitis caused by paints containing isothiazolinones is not rare, and may be severe and long-lasting. Better regulation of isothiazolinone concentrations in paints, and their adequate labelling, is urgently needed.

An alarming increase in contact allergy to methylisothiazolinone (MI) has been noted worldwide. Airborne exposure to MI in paint is an acknowledged occupational hazard. We present the first Australian report of airborne allergic contact dermatitis (ACD) following exposure to wall paint containing MI.

A 3-year-old girl with no history of atopic dermatitis was referred from a paediatric department because of recalcitrant perioral dermatitis of duration 10 weeks (Fig. 1); this had initially been treated as impetigo with systemic and topical antibiotics, with little or no effect. Topical hydrocortisone/miconazole had a limited effect. The patient was treated as an inpatient with topical fusidic acid and betamethasone 17-valerate (FucicortTM), which improved the dermatitis. Microbiological and fungal cultures were negative. Patch tests were performed with the European baseline series (TRUE TestTM; SmartPractice, Hillerod, Denmark) supplemented with the remaining baseline allergens in Finn Chambers® on Scanpor® tape (SmartPractice, Phoenix, AZ, USA). The patient was also tested with her own products. The tests showed positive reactions to methylisothiazolinone/methylisothiazolinone (++ on D3; + on D7) and methylisothiazolinone 0.02% aqua (+++ on D3). There was also a positive reaction to FucidinTM cream ‘as is’ (+ on D3), but there were no positive reactions to the FucidinTM ointment and fusidic acid sodium salt 2% pet. Airborne contact dermatitis and asthma in a nail art operator

Nail art (creation and decoration of artificial nails) is a growing fashion trend. Proportionally to its popularity, the number of allergic reactions to the materials used has recently increased. We report the case of a nail art operator who developed occupational allergy to acrylates, manifested with the unusual simultaneous presence of asthma and dermatitis. Cutaneous lesions only affected areas not covered by individual protection devices or clothes, even if such areas were not in direct contact with acrylates, suggesting airborne allergic contact dermatitis. While respiratory problems were solved with the correct use of a mask at the workplace, facial dermatitis proved impossible to avoid or control and, for this reason, the patient had to change her work.

An update on airborne contact dermatitis: 2007–2011

Reviews on irritant and allergic airborne contact dermatitis have been previously reported in the literature. Here, we present an update based on recently published airborne-induced skin reactions. For this survey, we screened the journals Contact Dermatitis, Dermatitis, and included relevant articles from other journals during the period January 2007 to December 2011. We also present the airborne cases observed in our department during the same time period. This survey provides an updated list of causal agents that have produced airborne allergic contact dermatitis, and briefly mentions some other types of skin reaction induced by airborne exposure. The sources of the reactions are multiple: drugs; plants, natural resins, and wood allergens; plastics, rubbers, and glues; preservatives and other chemicals; and metals. Airborne contact dermatitis is frequent, and most of the airborne allergens (and irritants) identified are in occupational settings. Drugs and preservatives have recently become more important causes. Dermatologists and occupational physicians need to be aware of them.

Airborne Allergic Contact Dermatitis From Tylosin in Pharmacy Compounders and Cross-Sensitization to Macrolide Antibiotics

Tylosin is a broad-spectrum macrolide antibiotic that is restricted to veterinary use. Allergic contact dermatitis (ACD) caused by tylosin has been reported in the literature from the farming industry and veterinary medicine. It is also reported as the most common antibiotic to cause ACD in the previously mentioned occupational settings. We present 2 cases of airborne ACD from tylosin among veterinary pharmaceutical compounding technicians. To our knowledge, only one other case of patch test-confirmed tylosin ACD has been reported in the manufacturing setting. Based on our results, cross-sensitization to other clinically relevant macrolides does not appear to be a concern. Our cases highlight the importance of patch testing among pharmaceutical compounders where the incidence of an airborne contact may be greater, given that the exposure is to the powdered form of potential allergens.

Airborne contact dermatitis - current perspectives in etiopathogenesis and management.

The increasing recognition of occupational origin of airborne contact dermatitis has brought the focus on the variety of irritants, which can present with this typical morphological picture. At the same time, airborne allergic contact dermatitis secondary to plant antigens, especially to Compositae family, continues to be rampant in many parts of the world, especially in the Indian subcontinent. The recognition of the contactant may be difficult to ascertain and the treatment may be even more difficult. The present review focuses on the epidemiological, clinical and therapeutic issues in airborne contact dermatitis.

Occupational Airborne Allergic Contact Dermatitis Caused by Gold

Airborne allergic contact dermatitis caused by gold is rare, often not even suspected and hence not investigated. Our purpose was to elucidate the clinical relevance of gold-induced airborne contact dermatitis. We analyzed and discussed the occupational dermatitis of two women working as restorers. Both patients' test results showed positive reactions to gold sodium thiosulfate only. The stop-and-relapse test confirmed the diagnostic hypothesis of airborne contact allergy. Our patients turned out to be gold sensitized because of occupational exposure to gold dust and gold leaf.

Study of pro‐ and anti‐inflammatory cytokine profile in the patients with parthenium dermatitis

Parthenium dermatitis is a common airborne allergic contact dermatitis induced by exposures to the weed Parthenium hysterophorus. The disease manifests as itchy erythematous papules, papulovesicular and plaque lesions on exposed areas of the body. The aim of this study was to show the alterations in pro/anti-inflammatory cytokines in parthenium dermatitis. The study included 50 patients with parthenium dermatitis confirmed by patch testing using aqueous extracts of P. hysterophorus and 50 age-matched healthy controls. The levels of pro-inflammatory [tumour necrosis factor-α (TNF-α), interleukin (IL)-6, IL-8, and IL-17] and anti-inflammatory (IL-4 and IL-10) cytokines were estimated by commercially available high sensitivity enzyme-linked immunosorbent assay (ELISA) kits. All the dermatitis patients showed significantly (P < 0.001) elevated levels of TNF-α, IL-6, IL-8, and IL-17 levels as compared to healthy controls. In contrast, the anti-inflammatory cytokine IL-4 showed an insignificant decrease (P < 0.217) and a decrease in level of IL-10 was statistically significant (0.001) compared with controls. The present study suggests the involvement of pro-inflammatory cytokines in the pathogenesis of parthenium dermatitis. A decrease in levels of anti-inflammatory cytokines was demonstrated, which could not downregulate pro-inflammatory cytokines in parthenium dermatitis.

Patients with airborne sensitization/contact dermatitis from budesonide‐containing aerosols ‘by proxy’

Inhalation corticosteroids (CSs), despite their wide use, rarely cause sensitization in subjects administering them. To determine the cause of sensitization and/or of allergic contact dermatitis which occurred in air-exposed body areas of patients reacting to corticosteroids and to budesonide, in particular. We reviewed the patch test results and sensitization sources in patients who reacted positively to corticosteroids tested in the K.U. Leuven Dermatology department during an 18-year period. Fifteen subjects, not themselves treated by budesonide-containing aerosols, but taking care of/or living together with patients who used them because of a chronic respiratory affection, appeared to have been sensitized by airborne exposure and/or presented with airborne allergic contact dermatitis from them. Air exposure to inhalation corticosteroids used 'by proxy' and to budesonide, in particular, needs to be taken into account as a potential cause of primary sensitization and/or airborne allergic contact dermatitis, sometimes also in an occupational context.
