Fatores associados à evolução clínica de pessoas idosas por Covid-19 segundo disponibilidade vacinal: estudo observacional

Objective: To identify the association of sociodemographic and clinical factors with the clinical outcomes of older adults hospitalized in intensive care units (ICUs) in the state of Paraná, according to the availability of COVID-19 vaccination. Method: Analytical observational, population-based study using secondary data from older adults hospitalized with COVID-19 in ICUs. Conditional odds ratios were estimated, homogeneity between strata was assessed using the Breslow-Day test (5%), and conditional independence was evaluated using the Mantel-Haenszel test, with estimation of the common odds ratio. Results: The following were associated with higher odds of hospital discharge: age group (OR = 1.97), female sex (OR = 1.20), and White race/color (OR = 1.22). Older adults with ≤ 9 years of schooling (OR = 0.63), risk factors (OR = 0.69), diabetes (OR = 0.82), immunodeficiency (OR = 0.57), liver disease (OR = 0.50), hematological disease (OR = 0.56), chronic lung disease (OR = 0.61), and use of ventilatory support (OR = 0.30) had lower odds of hospital discharge. Conclusion: Vaccination was associated with higher odds of hospital discharge according to age group, sex, White skin color, and higher educational level. Comorbidities such as diabetes mellitus, immunodeficiency, liver disease, chronic lung disease, and the use of ventilatory support reduced this likelihood.

Chatbot para identificação e monitoramento de toxicidades associadas à quimioterapia antineoplásica em pacientes com câncer

Objective: To develop a chatbot for the identification and monitoring of toxicities associated with intravenous antineoplastic chemotherapy in patients with cancer. Method: This methodological study was conducted between December 2023 and August 2025 and was based on the Interdisciplinary Method for the Development of Health Technologies, which comprises two phases and six activities. Phase 1 included Activity 1 – problem understanding and scoping review; Activity 2 – literature review; and Activity 3 – development of a low-fidelity prototype, prototype evaluation by nursing staff, and development of a high-fidelity prototype. Phase 2 comprised Activity 4 – development of the interactive prototype; Activity 5 – usability analysis and content validation; and Activity 6 – final writing and technology registration. Results were analyzed using the Content Validity Index (CVI > 90%) and usability assessment conducted with the System Usability Scale (score range, 0-100). Results: Nine symptoms most frequently reported in the literature supported the development of the chatbot. In the usability evaluation, 74.4% of users rated the system at the highest possible level of usability. In the content validation process, the achieved CVI was 90%. Conclusion: The chatbot demonstrated satisfactory content validity and usability, characterizing it as an appropriate technology to support health services in the remote monitoring of toxicities associated with chemotherapy, facilitating early identification and timely interventions.

In recent years, global science has witnessed an exponential expansion in the volume of published scientific articles. This scenario, driven by increasing pressures for research productivity and publication, has also been accompanied by a rise in the number of scientific article retractions, defined as the formal withdrawal of an already published article. Such retractions may occur due to the detection of honest errors or, more frequently, as a result of researcher misconduct, that is, the intentional violation of the principles of research honesty and integrity, including data fabrication, manipulation, or falsification; plagiarism; duplicate publication; and unethical research practices. These issues undermine the credibility of science, clinical decision-making, and the formulation of public policies (1) . This scenario has posed increasing challenges for researchers, organizations responsible for developing and promoting guidelines for different types of studies, and scientific journals committed to publication quality. Methodological rigor, transparency, reproducibility, and scientific integrity are globally valued principles that underpin trust in research. In this context, the development and prior dissemination of research protocols, whether for review studies or clinical research, constitute essential steps that prospectively outline the research plan, guide methodological decisions, reduce biases and potential errors, and promote transparency throughout the research process (2- 3) .

Diagnósticos de enfermagem em idosos com COVID-19 em uso de ventilação mecânica invasiva: um estudo longitudinal

Objective: To identify the NANDA International Nursing Diagnoses (ND) in older adults in intensive care for COVID-19 who required invasive mechanical ventilation support. Methods: A longitudinal observational study, based on the retrospective analysis of nursing records from 41 patient charts at three distinct moments: upon admission to the intensive care unit, 24 hours after orotracheal intubation, and at the moment before the outcome (discharge, death, or extubation). Odds ratios were calculated. The manuscript was guided by the STROBE statement. Results: With an average hospitalization time of 17 days, 127 NDs were identified at admission, 133 NDs 24 hours after intubation, and 159 NDs before the outcome. "Risk for Infection" had the highest frequency throughout hospitalization (53.7%, 75.6%, and 87.8%), while the diagnosis of "Anxiety" showed the largest negative variation (-240%). While "Anxiety" increased the odds of "Ineffective Airway Clearance" (7.5 times) and "Ineffective Breathing Pattern" (27.7 times), "Ineffective Breathing Pattern”, in turn, was associated with "Risk for Impaired Skin Integrity" (59 times). Conclusions: Different NDs were identified throughout the hospitalization of the older adults, with great variability in frequencies, depending on the clinical moment evaluated.

Análise microbiológica de cateteres centrais de inserção periférica e cateteres intravenosos periféricos curtos: um estudo transversal

Objective: To analyze the microbiological profile of peripherally inserted central catheters (PICC) and short peripheral intravenous catheters (PIVC). Method: A cross-sectional, quantitative study was conducted from April 2023 to October 2024. Removal of PIVC was indicated by signs suggestive of infection, phlebitis, or other clinical changes. Removal of PICC occurred per medical order or upon completion of therapy. Sociodemographic and clinical data were collected at the time of device removal. Results: A total of 30 PIVCs were analyzed, 12 of which (40%) showed microbial growth. The most prevalent organisms were Staphylococcus epidermidis and Staphylococcus haemolyticus. Both were resistant to oxacillin, gentamicin, levofloxacin, clindamycin, erythromycin, and rifampin. Of the 18 PICC tips, 5.55% yielded Staphylococcus capitis. A positive blood culture for Pseudomonas putida occurred in 5.55% of cases without corresponding growth on the catheter tip. Conclusion: Gram-positive bacteria predominated, with a higher occurrence in PIVC. Findings suggest that the patient’s diagnosis influences the identified microbiological profile.

Telenfermagem como estratégia de cuidado a crianças com cuidados contínuos e complexos após alta hospitalar

Objective: To explore the perceptions of nurses working in hospital pediatric settings and primary health care regarding the implementation of a telenursing service to follow children with continuous and complex care needs after discharge. Method: This is a qualitative study based on 18 semi-structured interviews with nurses from pediatric units of a university hospital and primary health care services in a city in Southern Brazil. Data were collected from March to June 2025 and analyzed using inductive thematic analysis. Results: Nurses viewed telenursing as a strategy that supports care for these children and noted the high prevalence of such cases in hospital settings. They reported structural and organizational barriers that hinder adherence, including workload burden, technological limitations, and families’ social vulnerability. Participants suggested establishing a municipal call center and implementing shared care across levels of the health system. Conclusion: Telenursing was considered feasible and innovative for caring for children with chronic and complex conditions; however, successful implementation requires investment in infrastructure and reorganization of the Health Care Network. The development of shared care models between hospital services and primary health care, strengthened counter-referral pathways, and clinical protocols is recommended to consolidate this model.

O letramento racial como proposta formativa crítica na enfermagem: uma reflexão

Introduction: This reflection article examines racial literacy (RL) as an educational tool to address ethnoracial inequalities in health care. Although the Black population is broadly represented within the Unified Health System, professional education still privileges hegemonic models that disregard ethnoracial diversity. In this context, Nursing emerges as a strategic field in promoting equity, particularly through continuing education. The aim is to critically discuss RL through the lens of bell hooks’ engaged pedagogy, highlighting its possibilities and challenges. Development: Drawing on bell hooks’ engaged pedagogy and decolonial frameworks, a four-dimensional educational pathway is proposed: i) Afro-centered theoretical foundation; ii) presentation of evidence; iii) conceptual deepening; and iv) implementation of concrete practices. The nurse’s role is emphasized as an articulator of knowledge and a promoter of critical, antiracist educational actions in the daily practice of interprofessional teams. Conclusion: Integrating RL into continuing health education requires political intentionality, ethical commitment, and an intersectional approach. By embracing this leading role, Nursing strengthens its identity as a social practice committed to transforming care relationships and fostering more equitable and inclusive institutional environments.

Validação transcultural para a língua portuguesa brasileira do instrumento Patient Evaluation of Emotional Comfort Experienced

Objective: To transculturally adapt the Patient Evaluation of Emotional Comfort Experienced (PEECE) instrument to Brazilian Portuguese. Method: The transcultural adaptation procedure for the instrument was performed following the process proposed by BEATON et al. (2000) and complying with stages I to V; in other words, the processes corresponding to translation, synthesis of the translations, back-translation, assessment by an experts' committee and testing the version with patients. The research was approved by a Research Ethics Committee under Opinion No. 6,456,631. Results: All stages proposed (1 to 4) were completed (Translation, Synthesis of the translations, Back-translation and Experts' committee), obtaining good results. The experts' committee reached 83.3%-100% agreement as for semantic, idiomatic, experiential and conceptual equivalence and the content validity index was 100% for theoretical relevance, pertinence and clarity. The assessment by the patients (Stage 5) revealed that 100% of them considered the overall appearance (structure, organization of the items and their answers) as good or optimal and that 96.7% rated readability as optimal. Conclusion: The version obtained after the assessment by the experts' committee showed conceptual equivalence to the original instrument.

Tendência empreendedora de enfermeiros na assistência a pessoas com feridas: estudo transversal

Objective: To assess the association between sociodemographic profile and entrepreneurial tendency among nurses providing care to individuals with wounds. Method: A cross-sectional study conducted in 2023 with 115 nurses who participated in an educational activity organized by the Regional Nursing Council of Rio de Janeiro. Data were collected virtually using the Entrepreneurial Tendency Test. Results: Entrepreneurial nurses were predominantly women (87.9%) over 41 years of age (62.6%), with postgraduate education (94.7%), more than 10 years since graduation (66.9%), and engaged in entrepreneurship to supplement their income (87.8%), which ranged between 4 and 9 minimum wages (64.5%). Most participants showed low or very low entrepreneurial tendency (40.1%). Higher scores in the success/achievement and creative tendency dimensions were associated with higher income levels. Conclusions: Although the nurses assessed demonstrated low or very low entrepreneurial tendency, they were experienced, highly qualified professionals who seek to advance their careers through drive and determination.
