Diarrhea is a frequent and potentially debilitating condition in immunocompromised pediatric patients. Gastrointestinal illnesses can present with prolonged symptoms and systemic and/or disseminated disease among children with immunocompromising conditions, such as solid organ transplantation, hematopoietic stem cell transplantation, or congenital immunodeficiencies. The increasing availability and use of molecular diagnostic testing has improved our understanding of the burden of specific pathogens causing gastrointestinal illness in immunocompromised children. Supportive care remains as the mainstay of therapy with immunosuppression reduction when feasible. Vaccine has been successful for prevention of rotavirus, and additional vaccines for other viral causes of gastroenteritis are needed.
