Postoperative Writing Impairment in Glioma Patients Undergoing Awake Craniotomy.

Awake craniotomy is increasingly used as the criterion standard for glioma resection in the perisylvian region to preserve language function. Perioperative language assessment of patients with glioma who underwent awake surgery was conducted to examine the characteristics of language impairment, including written language. The present study retrospectively analyzed cases of glioma involving language areas in patients who underwent awake craniotomy between April 2021 and December 2024. Background of the patients, surgical outcomes, and perioperative Japanese language functions, including speaking, listening, reading, and writing, were assessed using the Standard Language Test for Aphasia (SLTA). We collected data from 8 patients who underwent awake craniotomy, including 1 bilingual patient (Japanese and Vietnamese). An object naming task was used for intraoperative mapping in all cases. Pathological diagnoses included 4 low-grade gliomas and 4 glioblastomas. Subtotal resection was achieved in 2 cases, partial resection in 4 cases, and biopsy was performed in 2 cases. Postoperative assessments indicated both improvement and deterioration in language function across major domains, except for reading. Writing function declined in 3 of 5 patients with SLTA deficits. Younger patients and those without contrast-enhancing mass lesions showed a greater tendency toward writing deterioration, although this was not significant. Nevertheless, all patients eventually demonstrated improvement in writing function. Careful evaluation of not only speech, but also writing may be important, especially in younger patients and those without contrast-enhancing tumors. Incorporating attention to writing during awake mapping may help preserve broader language function and improve quality of life.

Janus kinase inhibitors (JAKi) are medicines used to treat inflammatory rheumatic musculoskeletal diseases (IRMDs) and inflammatory bowel diseases (IBDs). In Europe, JAKis received marketing authorisation in 2017 for rheumatoid arthritis. Subsequently, the European Medicines Agency (EMA) issued multiple safety alerts concerning this drug class. We aimed to examine how the use of JAKis developed in Finland. We conducted a retrospective time series analysis on the initiations and use of JAKis in the treatment of IRMD and IBD, utilising the national medicines reimbursement registry 2018-2023, including nearly all JAKi prescriptions. We assessed the initiators' age, line of treatment and atherosclerotic risk factors. Between 2018 and 2023, the prevalence of JAKi use increased from 0.14 to 0.43/1000 inhabitants, from 756 to 1740 among IRMD patients and from 11 to 672 among IBD patients. In IRMD, JAKis were rapidly adopted with 677 initiations in 2018, declining to 328 initiations in 2023. The corresponding numbers in IBD were 11 in 2018 and 358 in 2023. The mean age of IRMD patients initiating JAKis decreased from 54 years in 2021 to 41 years in 2023, while IBD patients' mean age changed from 42 to 39 years. In 2020, 52% of IRMD Patients Initiated JAKi in second-line of treatment but by 2023, this decreased to 31%. Prevalence of atherosclerotic risk factors declined. The uptake of JAKis among IRMD patients were initially fast. After safety alerts, initiation rates declined and shifted towards younger patients with fewer atherosclerotic risk factors. In contrast, the uptake of JAKis among IBD patients increased steadily.

Clinical trials on cochlear gene therapy for patients with autosomal recessive deafness 9 through transcanal and transmastoid approaches were reported in 2024. While both are clinically feasible in older patients, the small size of the external auditory canal (EAC) in younger patients may limit the applicability of transcanal endoscopic approaches. Therefore, we aimed to evaluate pediatric EAC anatomy by using temporal bone computed tomography (CT). Retrospective study. Tertiary center. Children aged 0 to 18 years. We analyzed 624 temporal bone CT scans obtained between 2017 and 2024. Measurements included the full, bony, and cartilaginous lengths of the posterior and inferior EAC walls, angle between the inferior EAC wall and tympanic membrane, diameter at the narrowest point of the EAC, and tympanic membrane diameter, all assessed using a standardized method. The full and bony lengths of the inferior EAC wall, the angle between the inferior EAC wall and tympanic membrane, and the diameter at its narrowest point increased dramatically between 0 and 1 year of age. During this period, the annual increases in these measurements were 2.21mm, 8.59mm, 33.0 degrees, and 1.43mm, respectively. In contrast, the tympanic membrane diameter remained unchanged throughout childhood. The feasibility of transcanal endoscopic ear surgery depends on the diameter at the narrowest point of the EAC, the endoscope and surgical instrument dimensions, and the length of the bony part of the EAC needed to elevate a tympanomeatal flap. Based on our findings, a transmastoid microscopic approach should be considered instead of transcanal approach in patients under 1 year of age.

A Mixed Methods Study of Anxiety and Patient Perception of Safety in Contrast-Enhanced MRI.

Contrast administration may contribute to patient anxiety and safety concerns during magnetic resonance imaging (MRI), yet patient experiences specific to contrast-enhanced MRI (CE-MRI) examination remain underexplored. To explore patients' experiences during CE-MRI examination, focusing on anxiety levels and safety perceptions. Questionnaire-based study with follow-up patient interviews. Two hundred and three adult patients undergoing CE-MRI in public hospitals; 28 participated in follow up interviews. 1.5 Tesla/standardized MRI scanning protocols across multiple body regions comprised 6-17 sequences per examination, with total table times ranging from 30 to 50 min, depending on the anatomical region examined. A structured questionnaire consisted of 31 items covering demographic characteristics, scan related information, experience with the CE-MRI examination, including information received, and anxiety assessment before, during, and after the CE-MRI examination using a four-point Likert scale. Following the CE-MRI examination, 28 patients from this original cohort participated in semi-structured interviews to explore their anxiety levels and perceptions of safety during the examination. Quantitative data were analyzed using SPSS, and a p value < 0.05 was considered significant. While qualitative data were analyzed thematically using NVivo software. Forty-six percent of patients reported increased anxiety during contrast administration, and 26.1% rated the information provided about the examination unclear. It was found that 35% felt the examination did not meet their expectations based on the information given. Anxiety was more prevalent among females (Mean = 1.71), younger patients (Mean = 2.04), and those with higher education levels (Mean = 1.96). Qualitative findings identified key anxiety triggers, including sensory discomfort, fear of unknown outcomes, and poor timing or poor clarity of information. Empathetic communication and clarity were perceived as essential anxiety-reducing factors. Notably, anxiety levels significantly decreased after the examination (χ2 = 24.955). Anxiety levels were influenced by sex, age, education level, the provided information, and the quality of communication. 4. Stage 3 (Diagnostic Thinking).

Although most imaging assessments are made qualitatively, quantitative measurements in orthopedic imaging are becoming more important in detecting subtle findings and assessing degrees of abnormality, which can help direct surgical management. In the knee, patellofemoral maltracking is a common cause of anterior pain, particularly in younger patients. Failure to recognize this pathophysiology may result in accelerated chondral loss and osteoarthritis (OA), and the imaging findings may be subtle. As a result, clinicians and radiologists have developed numerous measurements to detect and quantify patellofemoral alignment. Additional entities, such as femorotibial subluxation or angular abnormalities, may only be detected by using standardized measurements and can help detect ligamentous insufficiency or developmental malalignment, which can lead to instability and OA in the medial and lateral femorotibial compartments. Finally, the proper positioning and hardware selection for knee arthroplasty are critical to preventing early hardware failure and postsurgical pain. This review, which focuses on the knee, is the second in a three-part series discussing the appropriate imaging modalities on which to obtain specific lower extremity measurements as well as proper measurement techniques, grouped by pathology. Furthermore, the normal value or range of values according to current literature is reported, along with the significance of abnormal measurements.

<h2>Abstract</h2> Primary central nervous system lymphoma (PCNSL) is a rare and aggressive neoplasm usually of B-cell origin. Its occurrence is associated with age, with PCNSL affecting more often elderly patients. In adults, PCNSL is a non-germinal center B-cell lymphoma characterized by C5/MCD/MYD88 gene signature, and HLA I/II expression alterations. The disease behaviour differs in children, adolescent, and young adults (CAYA) or in those with an inborn or acquired immune dysregulation/dysfunction, with greater variety of histologic subtypes. Also, different genetic signatures are observed in younger patients (<i>MYD88</i> wildtype) or in those with EBV+ tumours, including higher frequency of abnormalities in checkpoint inhibitor axes, suggesting different tumour microenvironment compositions and immune escape mechanisms. Children, adolescents and young adults (CAYA) with PCNSL demonstrate more favourable outcomes, likely reflecting a superior response to therapy, whereas outcomes in adult patients remain comparatively poor. Focused research that specifically includes CAYA PCNSL is necessary to uncover additional unique molecular features and association with the immune system in an "immune-privileged" site such as the CNS. These efforts will likely lead to better diagnostic methods, prognostication and optimization of treatment protocols in children, and adolescents with PCNSL.

Comprehensive clinicopathological evaluation of malignant localized gingival enlargements in the Thai population.

Malignant localized gingival enlargements (LGEs) are rare and often clinically mimic reactive or benign lesions, creating diagnostic challenges. The present study aimed to evaluate the prevalence and origins of malignant LGEs, assess clinicopathological diagnostic concordance, and identify the distinction of clinical features in a Thai population. A total of 96 malignant LGEs diagnosed over a 40-year period were retrospectively analyzed. Demographic data, clinical characteristics, and clinical and pathological diagnoses were collected. Malignant LGEs were categorized by origin, and diagnostic concordance was evaluated. Clinical features were compared between epithelial and non-epithelial malignant LGEs, and a decision tree model was constructed to identify predictive factors. Epithelial (60.4%) and non-epithelial (39.6%) malignant LGEs indicated a comparable distribution. Non-epithelial lesions demonstrated substantially lower diagnostic concordance and were frequently misdiagnosed as reactive or benign conditions. Significant differences between groups were observed in age (P=0.004), surface texture (P=0.003), color (P=0.014), and location (P=0.044). Decision tree analysis identified age (P=0.018) and surface texture (P=0.03) as key predictors of non-epithelial malignant LGEs. Non-epithelial malignant LGEs are particularly prone to clinical misdiagnosis. Lesions occurring in younger patients, notably those with smooth surface texture, should raise suspicion for non-epithelial malignancy and prompt timely biopsy and referral. These findings may aid clinicians in improving early recognition of malignant LGEs.

Currently, prostate calculi are a common disease that is often encountered by urologists in their daily practice. In most cases, urologists do not attach clinical importance to prostate stones, detecting them accidentally during ultrasound examination against the background of pathologies such as prostatic hyperplasia, chronic prostatitis or vesiculitis. Performing surgical intervention in young patients is a complex task, the consequences of which can have a negative impact on the reproductive function of men. In the medical literature, only isolated cases of operations for prostate stones are described. This article presents a clinical case of surgical treatment of a patient who was found to have multiple large calculi of the prostate gland. Keywords: Transurethral laser lithotripsy, prostate stones, benign prostatic hyperplasia, chronic prostatitis.
