Deep learning-based multimodal fusion of the surface ECG and clinical features in prediction of atrial fibrillation recurrence following catheter ablation

BackgroundDespite improvement in treatment strategies for atrial fibrillation (AF), a significant proportion of patients still experience recurrence after ablation. This study aims to propose a novel algorithm based on Transformer using surface electrocardiogram (ECG) signals and clinical features can predict AF recurrence.MethodsBetween October 2018 to December 2021, patients who underwent index radiofrequency ablation for AF with at least one standard 10-second surface ECG during sinus rhythm were enrolled. An end-to-end deep learning framework based on Transformer and a fusion module was used to predict AF recurrence using ECG and clinical features. Model performance was evaluated using areas under the receiver operating characteristic curve (AUROC), sensitivity, specificity, accuracy and F1-score.ResultsA total of 920 patients (median age 61 [IQR 14] years, 66.3% male) were included. After a median follow-up of 24 months, 253 patients (27.5%) experienced AF recurrence. A single deep learning enabled ECG signals identified AF recurrence with an AUROC of 0.769, sensitivity of 75.5%, specificity of 61.1%, F1 score of 55.6% and overall accuracy of 65.2%. Combining ECG signals and clinical features increased the AUROC to 0.899, sensitivity to 81.1%, specificity to 81.7%, F1 score to 71.7%, and overall accuracy to 81.5%.ConclusionsThe Transformer algorithm demonstrated excellent performance in predicting AF recurrence. Integrating ECG and clinical features enhanced the models’ performance and may help identify patients at low risk for AF recurrence after index ablation.

Targeting the NLRP3 inflammasome signalling for the management of atrial fibrillation.

Inflammatory signalling via the nod-like receptor (NLR) family pyrin domain-containing protein-3 (NLRP3) inflammasome has recently been implicated in the pathophysiology of atrial fibrillation (AF). However, the precise role of the NLRP3 inflammasome in various cardiac cell types is poorly understood. Targeting components or products of the inflammasome and preventing their proinflammatory consequences may constitute novel therapeutic treatment strategies for AF. In this review, we summarise the current understanding of the role of the inflammasome in AF pathogenesis. We first review the NLRP3 inflammasome pathway and inflammatory signalling in cardiomyocytes, (myo)fibroblasts and immune cells, such as neutrophils, macrophages and monocytes. Because numerous compounds targeting NLRP3 signalling are currently in preclinical development, or undergoing clinical evaluation for other indications than AF, we subsequently review known therapeutics, such as colchicine and canakinumab, targeting the NLRP3 inflammasome and evaluate their potential for treating AF.

Prognosis and treatment strategies for atrial fibrillation in heart failure with mildly reduced ejection fraction.

The study investigates the prognosis of atrial fibrillation (AF) in patients with heart failure with mildly reduced ejection fraction (HFmrEF). Data concerning the prognostic impact of AF in patients with HFmrEF is scarce. Consecutive patients with HFmrEF [i.e. left ventricular ejection fraction 41-49% and signs and/or symptoms of heart failure (HF)] were retrospectively included at one institution from 2016 to 2022. Patients with AF were compared with patients without with regard to the primary composite endpoint of all-cause mortality and HF-related rehospitalization at 30 months (median follow-up). Statistical analyses included Kaplan-Meier, multivariable Cox proportional regression analyses, and propensity score matching. A total of 2148 patients with HFmrEF were included with an overall prevalence of AF of 43%. The presence of AF was associated with a higher risk of the primary composite endpoint all-cause mortality and HF-related rehospitalization at 30 months [hazard ratio (HR) = 2.068; 95% confidence interval (CI) 1.802-2.375; P = 0.01], which was confirmed after propensity score matching (HR = 1.494; 95% CI 1.216-1.835; P = 0.01). AF was an independent predictor of both all-cause mortality (HR = 1.340; 95% CI 1.066-1.685; P = 0.01) and HF-related rehospitalization (HR = 2.061; 95% CI 1.538-2.696; P = 0.01). Finally, rhythm control may be associated with lower risk of all-cause mortality compared with rate control for AF (HR = 0.342; 95% CI 0.199-0.587; P = 0.01). Atrial fibrillation affects 43% of patients with HFmrEF and represents an independent predictor of adverse long-term prognosis.

Association between aspirin and mortality in critically ill patients with atrial fibrillation: a retrospective cohort study based on mimic-IV database.

Atrial fibrillation (AF) is a prevalent issue among critically ill patients, and the availability of effective treatment strategies for AF is limited. The objective of this study was to evaluate the mortality rate associated with AF in critically ill patients who were either aspirin or non-aspirin users. This cohort study incorporated critically ill patients with AF from the Medical Information Mart for Intensive Care database. The study compared incidences of 28-day mortality, 90-day mortality, and 1-year mortality between patients with and without aspirin prescriptions. To assess the association between aspirin and the endpoints, Kaplan-Meier analysis and Cox proportional hazards regression analyses were conducted. In this study, a total of 13,330 critically ill patients with atrial fibrillation (AF) were included, of which 4,421 and 8,909 patients were categorized as aspirin and non-aspirin users, respectively. The 28-day, 90-day, and 1-year mortality rates were found to be 17.5% (2,330/13,330), 23.9% (3,180/13,330), and 32.9% (4,379/13,330), respectively. The results of a fully-adjusted Cox proportional hazard model indicated that aspirin use was negatively associated with the risk of death after adjusting for confounding factors (28-day mortality, HR 0.64, 95% CI 0.55-0.74; 90-day mortality, HR 0.65, 95% CI 0.58-0.74; 1-year mortality, HR 0.67, 95%CI 0.6∼0.74). The results of the subgroup analysis indicate a more robust correlation, specifically among patients under the age of 65 and those without a history of congestive heart failure or myocardial infarction. The utilization of aspirin may exhibit a correlation with a reduction in risk-adjusted mortality from all causes in critically ill patients diagnosed with atrial fibrillation. However, additional randomized controlled trials are necessary to elucidate and confirm this potential association.

Cognitive impairment and treatment strategy for atrial fibrillation in older adults: The SAGE-AF study.

Cognitive impairment is strongly associated with atrial fibrillation (AF). Rate and rhythm control are the two treatment strategies for AF and the effect of treatment strategy on risk of cognitive decline and frailty is not well established. We sought to determine how treatment strategy affects geriatric-centered outcomes. The Systematic Assessment of Geriatric Elements-AF (SAGE-AF) was a prospective, observational, cohort study. Older adults with AF were prospectively enrolled between 2016 and 2018 and followed longitudinally for 2 years. In a non-randomized fashion, participants were grouped by rate or rhythm control treatment strategy based on clinical treatment at enrollment. Baseline characteristics were compared. Longitudinal binary mixed models were used to compare treatment strategy with respect to change in cognitive function and frailty status. Cognitive function and frailty status were assessed with the Montreal Cognitive Assessment Battery and Fried frailty phenotype tools. 972 participants (mean age = 75, SD = 6.8; 49% female, 87% non-Hispanic white) completed baseline examination and 2-year follow-up. 408 (42%) were treated with rate control and 564 (58%) with rhythm control. The patient characteristics of the two groups were different at baseline. Participants in the rate control group were older, more likely to have persistent AF, prior stroke, be treated with warfarin and have baseline cognitive impairment. After adjusting for baseline differences, participants treated with rate control were 1.5 times more likely to be cognitively impaired over 2 years (adjusted OR: 1.47, 95% CI:1.12, 1.98) and had a greater decline in cognitive function (adjusted estimate: -0.59 (0.23), p < 0.01) in comparison to rhythm control. Frailty did not vary between the treatment strategies. Among those who had 2-year follow-up in non-randomized observational cohort, the decision to rate control AF in older adults was associated with increased odds of decline in cognitive function but not frailty.

Aggressive Rhythm Control Strategy in Atrial Fibrillation Patients Presenting at the Emergency Department: The HEROMEDICUS Study Design and Initial Results.

Atrial fibrillation has progressively become a more common reason for emergency department visits, representing 0.5% of presenting reasons. Registry data have indicated that about 60% of atrial fibrillation patients who present to the emergency department are admitted, emphasizing the need for more efficient management of atrial fibrillation in the acute phase. Management of atrial fibrillation in the setting of the emergency department varies between countries and healthcare systems. The most plausible reason to justify a conservative rather than an aggressive strategy in the management of atrial fibrillation is the absence of specific guidelines from diverse societies. Several trials of atrial fibrillation treatment strategies, including cardioversion, have demonstrated that atrial fibrillation in the emergency department can be treated safely and effectively, avoiding admission. In the present study, we present the epidemiology and characteristics of atrial fibrillation patients presenting to the emergency department, as well as the impact of diverse management strategies on atrial-fibrillation-related hospital admissions. Lastly, the design and initial data of the HEROMEDICUS protocol will be presented, which constitutes an electrophysiology-based aggressive rhythm control strategy in patients with atrial fibrillation in the emergency department setting.

One-year outcomes of anterior–posterior vs. anterior-lateral method of cardioversion for atrial fibrillation: a tertiary center experience

This retrospective cohort study aimed to compare the one-year outcomes of anterior–posterior (AP) and anterior—lateral (AL) methods of cardioversion for atrial fibrillation (AF). A total of 2168 patients were included, with 1125 patients in the AP cardioversion group (Group 1) and 1043 patients in the AL cardioversion group (Group 2). Baseline characteristics, primary and secondary outcomes, safety outcomes, and logistic regression predictors of sinus rhythm were analyzed. The results showed comparable rates of maintaining sinus rhythm at the one-year follow-up between the two groups (65.8% in Group 1 vs. 65.7% in Group 2, p = 0.042). There were no significant differences in the incidence of AF recurrence or safety outcomes between the groups. Logistic regression analysis identified the duration of AF and the presence of coronary artery disease as significant predictors of sinus rhythm maintenance. Additionally, the use of the AL method was associated with a higher likelihood of AF recurrence compared to the AP method (p = 0.043). These findings suggest that both the AP and AL methods of cardioversion are effective in achieving and maintaining sinus rhythm in AF patients. The duration of AF and the presence of coronary artery disease should be considered when selecting the cardioversion approach. These results contribute to the understanding of optimal treatment strategies for AF and support personalized management decisions based on individual patient characteristics.

Impact of Various Atrial Fibrillation Treatment Strategies on Length of Stay in the Emergency Department and Early Complications-3 Years of a Single-Center Experience.

Atrial fibrillation (AF) is the most common arrhythmia presenting in emergency departments (EDs), vastly increasing mainly due to society's lifestyles leading to numerous comorbidities. Its management depends on many factors and is still not unified. Aims: The aim of this study was to compare different AF management strategies in the ED and to evaluate their influence on the length of stay (LOS) in the ED and their safety. We analyzed medical records over 3 years of data collection, including age, primary AF diagnosis, an attempt to restore sinus rhythm, complications, and length of stay. Patients were divided into three groups according to the treatment method received: only pharmacological cardioversion (MED), only electrical cardioversion (EC), and patients who received medications followed by electrical cardioversion (COMB). We included 599 individuals in the analysis with a median age of 71. The restoration of sinus rhythm and LOS were as follows: MED: 64.95%, 173 min; COMB: 87.91%, 295 min; SH: 92.40%, 180 min. The difference between the MED and EC strategies, as well as MED and COMB, was statistically significant (p < 0.001 in both). The total number of complications was 16, with a rate of 32.67%. The majority of them followed a drug administration, and the most common complication was bradycardia. Electrical cardioversion is a safe and effective treatment strategy in stable patients with AF in the ED. It is associated with a shortened LOS. Medication administration preceded the majority of complications.

Myocardial injury and inflammation following pulsed-field ablation and very high-power short-duration ablation for atrial fibrillation.

Pulmonary vein isolation (PVI) using radiofrequency ablation (RFA) is an established treatment strategy for atrial fibrillation (AF). To improve PVI efficacy and safety, high-power short-duration (HPSD) ablation and pulsed-field ablation (PFA) were recently introduced into clinical practice. This study aimed to determine the extent of myocardial injury and systemic inflammation following PFA, HPSD, and standard RFA using established biomarkers. We included 179 patients with paroxysmal AF receiving first-time PVI with different ablation technologies: standard RFA (30-40 W/20-30 s, n = 52), power-controlled HPSD (70 W/5-7 s, n = 60), temperature-controlled HPSD (90 W/4 s, n = 32), and PFA (biphasic, bipolar waveform, n = 35). High-sensitivity cardiac troponin T (hs-cTnT), creatine kinase (CK),CK MB isoform(CK-MB), and white blood cell (WBC) count were determined before and after ablation. Baseline characteristics were well-balanced between groups (age 63.1 ± 10.3 years, 61.5% male). Postablation hs-cTnT release was significantly higher with PFA (1469.3 ± 495.0 ng/L), HPSD-70W (1322.3 ± 510.6 ng/L), and HPSD-90W (1441.2 ± 409.9 ng/L) than with standard RFA (1045.9 ± 369.7 ng/L; p < .001). CK and CK-MB release was increased with PFA by 3.4-fold and 5.8-fold, respectively, as compared to standard RFA (p < .001). PFA was associated with the lowest elevation in WBC (Δ1.5 ± 1.5 × 109 /L), as compared to standard RFA (Δ3.8 ± 2.5 × 109 /L, p < .001), HPSD-70W (Δ2.7 ± 1.7 × 109 /L, p = .037), and HPSD-90W (Δ3.6 ± 2.5 × 109 /L, p < .001). Among the four investigated ablation technologies, PFA was associated with the highest myocardial injury and the lowest inflammatory reaction.

Atrial fibrillation in patients with chronic kidney disease: features of pathogenesis and treatment

Atrial fibrillation (AF) is the most commonly diagnosed cardiac arrhythmia in adults, the frequency of which increases in patients with chronic kidney disease (CKD). The substrate for the development of AF is atrial cardiomyopathy, which includes structural, electrophysiological and molecular remodeling of the atria. AF, in turn, can initiate and accelerate the progression of CKD. Such a bidirectional relationship causes a frequent combination of these two conditions, leading to both a prothrombotic state and an increased risk of bleeding. In patients with CKD, the pharmacokinetics of drugs used in AF are changing, what limits their use in CKD S4/S5. If previously patients with CKD S4-5 were excluded from randomized clinical trials (RCTs) on treatment strategies for AF, a number of such studies on their management have been published to date. The purpose of the article is to review existing ideas about the features of the pathogenesis of AF in CKD and strategies of recent years for the treatment of AF with advanced stages of CKD.

Antiarrhythmic Drugs vs. Ablation for Atrial Fibrillation

Summary:&#x0D; Atrial fibrillation (AF) is a common and potentially life-threatening cardiac arrhythmia that affects millions of individuals worldwide. The treatment landscape for AF has traditionally comprised antiarrhythmic drugs (AADs), which aim to restore and maintain normal heart rhythm. However, over the past two decades, catheter ablation has emerged as an alternative therapeutic approach in the management of AF. This article systematically compares the effectiveness, safety, and long-term outcomes of AADs and catheter ablation as treatment strategies for atrial fibrillation.&#x0D; Introduction:&#x0D; Atrial fibrillation (AF) is characterized by disorganized electrical activity in the atria, causing irregular and often rapid heart rhythm. Over the years, pharmacological therapy with antiarrhythmic drugs (AADs) has been the mainstay of AF treatment. Nonetheless, catheter ablation has gained significant momentum due to its ability to directly target the arrhythmogenic substrate within the heart.1&#x0D; Efficacy:&#x0D; &#x0D; Antiarrhythmic Drugs: Various classes of AADs inhibit specific ion channels or receptors to normalize cardiac electrical conduction. While AADs can effectively restore sinus rhythm in many patients, their long-term success in maintaining sinus rhythm is modest, with recurrence rates ranging from 40% to 60% within one year.2&#x0D; Catheter Ablation: Ablation procedures selectively target and destroy the abnormal electrical pathways contributing to AF. In well-selected patients, catheter ablation has demonstrated higher efficacy rates than AADs, particularly in paroxysmal AF, with success rates reaching 70-80% or more after a single procedure.2&#x0D; &#x0D; Safety Profile:&#x0D; &#x0D; Antiarrhythmic Drugs: Although generally well-tolerated, AADs have notable adverse effects, including pro-arrhythmia, organ toxicity, and drug-drug interactions. These side effects often limit their usage or require careful patient monitoring.3&#x0D; Catheter Ablation: While catheter ablation is generally safe, it has a finite risk of procedural complications, such as vascular injury, cardiac perforation, and stroke. Nonetheless, the overall complication rate is low, ranging from 1% to 5%, and is further decreasing with advancements in technology and operator experience.3&#x0D; &#x0D; Quality of Life and Symptom Control:&#x0D; &#x0D; Antiarrhythmic Drugs: Effective rate control with AADs can significantly improve symptom burden and quality of life in patients with persistent or permanent AF.4&#x0D; Catheter Ablation: Successful ablation procedures can offer long-term freedom from AF symptoms, eliminating or reducing the dependence on AADs and their associated side effects.4&#x0D; &#x0D; Long-Term Outcomes:&#x0D; &#x0D; Antiarrhythmic Drugs: AAD therapy is often lifelong, and patients may require multiple medication adjustments due to loss of efficacy or intolerable side effects. However, AADs remain a treatment option for patients with contraindications for ablation or in those who prefer medical management.4&#x0D; Catheter Ablation: Successful ablation procedures have been associated with excellent long-term outcomes, with sustained freedom from AF achieved in approximately 60-70% of patients at 1-5 years of follow-up. Repeat ablations may be necessary in some cases.4&#x0D; &#x0D; Conclusion:&#x0D; While both antiarrhythmic drugs and catheter ablation have their advantages and disadvantages in the management of atrial fibrillation, catheter ablation has emerged as a promising treatment strategy, offering higher success rates and long-term symptom control compared to AADs. However, individual patient characteristics, preferences, and comorbidities should guide the selection of the optimal treatment approach, ensuring personalized and comprehensive management of atrial fibrillation. Catheter ablation in Asian patients with atrial fibrillation and hypertrophic cardiomyopathy: electrophysiological characteristics of recurrence and long-term clinical outcomes.

Catheter ablation (CA) is a treatment strategy for atrial fibrillation (AF) in patients with hypertrophic cardiomyopathy (HCM). We investigated the electrophysiological characteristics of recurrence in a tertiary referral center and compared long-term clinical outcomes after CA therapy with patients who did not undergo CA. Patients with HCM and AF who underwent CA (group 1, n = 60) or pharmacological treatment (group 2, n = 298) between 2006 and 2021 were enrolled in this study. The baseline characteristics and electrophysiological characteristics of group 1 patients were examined to elucidate the reason for the recurrence of AF after CA therapy. The clinical results of the patients in Group 1 and Group 2 were compared using a propensity score (PS)-matched method. The most common cause of recurrence was pulmonary vein reconnection (86.5%), followed by non-pulmonary vein triggers (40.5%), cavotricuspid isthmus flutter (29.7%), and atypical flutter (24.3%). Thyroid disease (HR, 14.713; P < 0.01), diabetes (HR, 3.074; P = 0.03), and non-paroxysmal AF (HR, 4.012; P = 0.01); these factors independently predicted recurrence. After the first recurrence, patients who underwent repeat CA showed a better arrhythmia-free state (74.1%) than those who underwent drug escalation therapy (29.4%, P < 0.01). After matching, PS-group 1 patients showed significantly better outcomes in all-cause mortality, heart failure hospitalization, and left atrial reverse remodeling than PS-group 2 patients. Patients who underwent CA showed better clinical outcomes than those who underwent drug therapy. The main predictors of recurrence were thyroid disease, diabetes, and non-paroxysmal AF.

PO-02-158 PACE AND ABLATE IS AN EFFECTIVE MANAGEMENT STRATEGY FOR ATRIAL FIBRILLATION IN PATIENTS WITH HYPERTROPHIC CARDIOMYOPATHY

Atrial fibrillation (AF) is common in patients with hypertrophic cardiomyopathy (HCM). In this cohort, the sequel of rapid ventricular response and symptoms associated with an irregular rhythm can be challenging to manage with medical therapy and catheter ablation (CA). Pace and ablate (PAb) strategy may offer an effective treatment alternative. Assess the efficacy of PAb in patients with HCM who have failed medical therapy and/or catheter ablation for AF. Single centre study with retrospective analysis of a prospectively collated HCM registry. PAb patients were identified. Baseline characteristics, procedural indication and procedural data was collected and symptoms defined by NYHA and EHRA classification plus echocardiographic findings in follow-up assessed. Forty-two patients (17 (40%) male and age 67±11 years) were included (Table 1) of which 18 (43%) patients had undergone previous CA ablation. Indications for PAb: 5 (12%) inappropriate shock, 24 (57%) ineffective rate control and 13 (31%) to regularise rhythm to improve symptoms. Procedural parameters: procedure time 70±12mins and immediate success in 40 patients (95%) with no complications. During a follow-up time of 62±54 months, left ventricular ejection fraction (LVEF) post PAb remained stable (LVEF Pre 52±10%, LVEF Post 50±9%, p=0.6). Those without a CRT also had no change in LV function during follow-up post PAb (LVEF pre 55±8% vs post 51±19%, p=0.21). Thirty-three (83%) patients reported an improvement in symptoms post procedure. In patients with symptom improvement there was no difference in AF type (14/15, 93% paroxysmal vs. 19/25, 76% persistent AF; p=0.22), presence of at least moderate mitral regurgitation (MR) (12/33, 36% moderate MR. 1/7, 14% no moderate MR; p=0.39), LV impairment (10/11, 91% LVEF <50% vs. 23/29, 79% LVEF ≥50%; p=0.65) or CRT device in situ (22/24, 92% CRT vs. 11/16, 69% no CRT; p=0.09). Symptoms improved in 10 (76%) patients who underwent PAb to regularise rhythm. In the majority of patient with HCM and AF, PAb improves symptoms without impacting LV function regardless of AF type, device implanted or the presence of pre-existing LV impairment. PAb was also shown to effectively improve symptoms through regularising the rhythm. Post PAb, LV function remains stable regardless of the type of device implanted. PAb is an effective treatment strategy for AF in patients with HCM.

The Impact of Atrial Fibrillation Treatment Strategies on Cognitive Function.

There is increasing evidence to suggest that atrial fibrillation is associated with a heightened risk of dementia. The mechanism of interaction is unclear. Atrial fibrillation-induced cerebral infarcts, hypoperfusion, systemic inflammation, and anticoagulant therapy-induced cerebral microbleeds, have been proposed to explain the link between these conditions. An understanding of the pathogenesis of atrial fibrillation-associated cognitive decline may enable the development of treatment strategies targeted towards the prevention of dementia in atrial fibrillation patients. The aim of this review is to explore the impact that existing atrial fibrillation treatment strategies may have on cognition and the putative mechanisms linking the two conditions. This review examines how components of the 'Atrial Fibrillation Better Care pathway' (stroke risk reduction, rhythm control, rate control, and risk factor management) may influence the trajectory of atrial fibrillation-associated cognitive decline. The requirements for further prospective studies to understand the mechanistic link between atrial fibrillation and dementia and to develop treatment strategies targeted towards the prevention of atrial fibrillation-associated cognitive decline, are highlighted.

Cost-Effectiveness Mathematical Model to Evaluate the Impact of Improved Cardiac Ablation Strategies for Atrial Fibrillation Treatment

Atrial fibrillation (AF) is the most common form of cardiac arrhythmia. Despite the frequency of the disease, the treatment strategies for AF are inefficient. We developed a cost-effectiveness model to evaluate potential improvements in the application of cardiac ablations to treat AF. These are surgical procedures to terminate the arrhythmia and restore Sinus Rhythm. A Markov Model with a time horizon of five years was built to represent the management of patients in AF. A Montecarlo simulation was developed as a sensitivity analysis when the effectiveness increases the estimate of the potential impact of an improvement on the efficacy of cardiac ablation. The result of the analysis showed 44% of patients were untreated in any way. The base case ends up with 45% of patients having sinus rhythm restored after five years. The Montecarlo simulation estimates that in 58% of cases, the alternative of increasing ablation effectiveness by 25% would be cost-effective. If the number of performed ablations is doubled, the robustness increases to 86%. In conclusion, the model of management of AF highlights the importance of not only increasing effectiveness, but also treating more patients. Our study shows that investing in new screening technology to increase the effectiveness of ablations would be cost-effective.

Superior mesenteric artery embolism after radiofrequency ablation in regularly anticoagulated patients with paroxysmal atrial fibrillation: a case report

BackgroundSuperior mesenteric artery embolism (SMAE) is a rare cause of acute abdomen, and the fatality rate is extremely high if it is not diagnosed and treated in time. Due to the lack of knowledge and experience of nonspecialist physicians, it is easy to misdiagnose. Radiofrequency ablation (RFA) has become the first-line treatment strategy for atrial fibrillation (AF). Thromboembolic events are some of the major complications after RFA, whereas SMAE is rarely reported.Case presentationA 70 year-old woman with paroxysmal AF who regularly took anticoagulant drugs for 3 months experienced abdominal pain after RFA. At the outset, she was misdiagnosed as mechanical intestinal obstruction. When the patient presented with blood in the stool, abdominal enhancement computed tomography was conducted and showed a small bowel perforation. Immediate laparotomy was performed, and the final diagnosis was SMAE.ConclusionIt is suggested that for unexplained abdominal pain after RFA of AF, the possibility of SMAE should be considered, and a targeted examination should be carried out in time to confirm the diagnosis and give appropriate treatment.

Thermal ablation induced by low-intensity ultrasound for pulmonary vein isolation

Atrial fibrillation (AF) is currently the most common arrhythmia disease in clinic and its incidence increases with the age, which seriously endangers peoples health. Percutaneous catheter ablation is an important treatment strategy for AF, and pulmonary vein (PV) isolation has been widely accepted as the key approach of AF ablation since AFs are regarded the most important trigger of AF. The ablation scheme based on low-intensity ultrasound has the advantages of low biotoxicity and minimal invasion, while its applicability and parameter settings are still not well known. In this paper, we proposed a finite element model that could be used to simulate the ablation application of ultrasound intervention device for the purpose of PV isolation. The acoustic pressure and temperature fields generated during the process of low-intensity ultrasound pulmonary vein ablation were simulated based on linear acoustic wave equation and Pennes biological heat transfer equation, and then the corresponding thermal damage was quantitatively analyzed by taking into account of the equivalent heat dose equation. The results showed that the incident acoustic energy was mainly concentrated in the blood vessels of PV, and the temperature rise was mainly constrained in the inner part of the vessel walls, which suggested that high temperature rise could be realized nearby the central region of the low-intensity ultrasound transducer to achieve significant thermal damage in the inner part of the vessel walls. Moreover, the current study also discussed the impacts of different parameters on the ultrasound ablation effect, which would provide theoretical support and guidance for the optimization of ultrasound ablation strategy in clinics.

Immune-associated pivotal biomarkers identification and competing endogenous RNA network construction in post-operative atrial fibrillation by comprehensive bioinformatics and machine learning strategies.

Atrial fibrillation (AF) is the most common arrhythmia. Previous studies mainly focused on identifying potential diagnostic biomarkers and treatment strategies for AF, while few studies concentrated on post-operative AF (POAF), particularly using bioinformatics analysis and machine learning algorithms. Therefore, our study aimed to identify immune-associated genes and provide the competing endogenous RNA (ceRNA) network forPOAF. Three GSE datasets were downloaded from the GEO database, and we used a variety of bioinformatics strategies and machine learning algorithms to discover candidate hub genes. These techniques included identifying differentially expressed genes (DEGs) and circRNAs (DECs), building protein-protein interaction networks, selecting common genes, and filtering candidate hub genes viathree machine learning algorithms. To assess the diagnostic value, we then created the nomogram and receiver operating curve (ROC). MiRNAs targeting DEGs and DECs were predicted using five tools and the competing endogenous RNA (ceRNA) network was built. Moreover, we performed the immune cell infiltration analysis to better elucidate the regulation of immune cells in POAF. We identified 234 DEGs (82 up-regulated and 152 down-regulated) of POAF via Limma, 75 node genes were visualized via PPI network, which were mainly enriched in immune regulation. 15 common genes were selected using three CytoHubba algorithms. Following machine learning selection, the nomogram was created based on the four candidate hub genes. The area under curve (AUC) of the nomogram and individual gene were all over 0.75, showing the ideal diagnostic value. The dysregulation of macrophages may be critical in POAF pathogenesis. A novel circ_0007738 was discovered in POAF and the ceRNA network was eventually built. We identified four immune-associated candidate hub genes (C1QA, C1R, MET, and SDC4) for POAF diagnosis through the creation of a nomogram and evaluation of its diagnostic value. The modulation of macrophages and the ceRNA network may represent further therapy methods.

Staged hybrid totally thoracoscopic maze and catheter ablation for atrial fibrillation.

IntroductionAtrial fibrillation (AF) is a growing health problem and is associated with increased risk of stroke. The Cox‐Maze surgical procedure has offered the highest success rate, but utilization of this technique is low due to procedure invasiveness and complexity. Advances in catheter ablation and minimally invasive surgical techniques offer new options for AF treatment.MethodsIn this review, we describe current trends and outcomes of minimally invasive treatment of persistent and long‐standing persistent AF.ResultsTreatment of persistent and long‐standing persistent AF can be successfully treated using a team approach combining cardiac surgery and electrophysiology procedures. With this approach, the 1‐year freedom from AF off antiarrhythmic drugs was 85%.DiscussionThere are a variety of techniques and approaches used around the world as technology evolves to help develop new treatment strategies for AF. Our report will focus on a hybrid treatment approach using surgical and electrophysiology approaches providing enhanced treatment options by replicating Cox‐Maze IV lesions using skills from each specialty. Closure of the left atrial appendage as part of these procedures enhances protection from late stroke. A team approach provides a cohesive evaluation, treatment, and monitoring plan for patients. Development of successful, less invasive treatment options will help address the growing population of patients with AF.

Cognitive impairment, age, quality of life, and treatment strategy for atrial fibrillation in older adults: The SAGE-AF study.

Atrial fibrillation (AF) treatment includes anticoagulation for high stroke risk individuals and either rate or rhythm control strategies. We aimed to investigate the impact of age, geriatric factors, and medical comorbidities on choice of rhythm versus rate control strategy in older adults. Patients with AF aged ≥65 years with CHA2 DS2 VASc score ≥2 and eligible for anticoagulation were recruited for the Systematic Assessment of Geriatric Elements-AF (SAGE-AF) prospective cohort study. An interview that included measures of HRQoL, cognitive function, vision, hearing, and frailty was performed. The association between these elements and AF treatment strategy was examined by multivariable logistic regression models. One thousand two hundred forty-four participants (mean age 76 years; 49% female; 85% non-Hispanic white) were enrolled. Rate and rhythm control were used in 534 and 710 participants, respectively. Compared to participants <75 years, those ≥75 were more likely to be treated with a rate control strategy (age 75-84 adjusted odds ratio [aOR] 1.37 [95% CI 0.99, 1.88]; age 85+ aOR=2.05, 95% CI 1.30, 3.21). Those treated with a rate control strategy were more likely to have cognitive impairment (aOR=1.50, 95% CI 1.13, 1.99), and peripheral vascular disease (PVD) (aOR=1.82, 95% CI 1.22, 2.72) but less likely to have visual impairment (aOR 0.73 [0.55, 0.98]), congestive heart failure (CHF; aOR 0.68 [0.49, 0.94]) or receive anticoagulation (aOR 0.53, 95% CI 0.36, 0.78). Older age, cognitive impairment, and PVD were associated with use of rate control strategy. Visual impairment, CHF, and anticoagulation use were associated with a rhythm control strategy. There was no difference in HRQoL between the rate and rhythm control groups. This study suggests that certain geriatric elements may be associated with AF treatment strategies. Further study is needed to evaluate how these decisions affect outcomes.
