Summary Seven cases of phlegmasia cerulea dolens are reported. This condition was characterized by severe pain, massive edema, mottled cyanosis, and loss of pulses. An associated condition, such as malignant disease, postoperative state, postpartum state, trauma, and infection, was frequent. The basic pathologic condition is massive obstructing venous thrombosis. The accompanying arterial insufficiency is mechanical, and vasospasm plays little or no role. The treatment of choice is early thrombectomy when the patient's general condition is satisfactory. In desperately ill patients elevation, anticoagulants, and sympathetic block may achieve satisfactory results. Relieving fasciotomy may be of aid. Lastly, surgical amputation should be attempted as a life-saving procedure.
