Perioperative Nursing Considerations for Transurethral Resection Prostatectomy

The present article was an overview of transurethral resection prostatectomy (TURP) and its prevalence, emphasizing the critical role of perioperative nursing in enhancing patient safety and outcomes. Preoperative nursing assessment was explored, covering the importance of patient history and risk assessment, diagnostic tests and evaluations, patient counseling, and informed consent, as well as preoperative optimization and preparation. The intraoperative nursing management section outlines key responsibilities in setting up the operating room, maintaining infection control, preventing specific clinical risk of TURP, managing anesthesia, ensuring surgical team coordination and communication, and adhering to evidence-based guidelines. Postoperative nursing care focused on immediate recovery and monitoring, pain management strategies, urinary catheter care, early ambulation, complication prevention, and patient education. Emerging trends and innovations in perioperative nursing were discussed, including advancements in surgical techniques, integration of new technologies, telemedicine, and the adoption of holistic care approaches. The present article also addressed the challenges facing perioperative nurses, such as workforce shortages and burnout, research gaps in TURP nursing, and the importance of promoting ongoing education and training. By examining these key elements, the present article provided a comprehensive resource for perioperative nurses, healthcare providers, and researchers, aimed to enhance understanding, improved patient care, and stimulated further advancements in the field.

Frequency of Prostatic Adenocarcinoma in Transurethral Resection of Prostatectomy Done for Benign prostatic Enlargement and Correlation with Serum Level of PSA

Background: Around the world, transurethral resection of the prostate, or TURP, is the most often used surgical procedure for treating benign prostatic hyperplasia (BPH). The current study is designed to evaluate the Frequency of prostatic Adenocarcinoma in transurethral resection of prostatectomy and its correlation with serum levels of PSA. Methods: The Retrospective study was conducted in the Histopathology Lab of Dr. Akbar Niazi Teaching Hospital Islamabad which is an PMDC-approved and ISO-9001-certified hospital. The data for this study was obtained by including consecutive cases of transurethral resection prostatectomy received for analysis in the histopathology lab during the study period. The data was retrieved from Histopathology record files. The slides were viewed and the diagnoses were confirmed. The patient’s serum PSA levels were retrieved from pre-operation hospital medical records. The Patients who presented to the histology lab for diagnosis were reassessed using a Questionnaire based on Performa divided into different sections such as socio-demographic data, reasons, History of Diseases, PSA level, Age, and complications. The collected data was analyzed using SPSS Version 21. Results: In 53 patients 11 were of Prostatic Adenocarcinoma, 14 were benign prostatic hyperplasia and 28 were prostatitis. with number and percentage. Prostate Specific Antigen value was different in patients. The highest reported value was 28ng/ml. The mean was 2.68 ng/ml. Most cases were present between the age group of 50-60 years with 32.1% Prostatic adenocarcinoma frequency was higher in the older age group levels above 4ng/ml were seen in prostatitis and BPH. Conversely, there were cases of prostatic adenocarcinoma with less than/ml PSA levels Conclusion: PSA levels were raised, above 4ng/ml in 8 cases while low in 3 cases of Prostatic adenocarcinoma. PSA levels were also raised in a case of prostatitis and two cases of BPH. We conclude that PSA levels although suggestive are not entirely specific for prostatic adenocarcinoma.

Radiological Evaluation of Lower Genitourinary Tract Cysts in Males

Aim of the Work: The present wok aims to assess how to reach the optimum diagnosis of the nature and the complications of lower genitourinary tract cyst in male patient using different radiological imaging modalities. Patients and Methods: 1000 male patients were evaluated with pelvic ultrasound and if lower genitourinary tract cysts were present, the patients were further subjected to either trans-rectal ultrasound (TRUS), TRUS guided aspiration for presence of spermatozoa, TRUS guided seminal vesiculography or CT urography in order to reach the origin of the cyst and the complications of its presence. Results: 14 cases were detected with lower genitourinary tract cysts among our patients with an overall incidence 1.4%; among them 3 patients with mid line prostatic cysts showed normal seminal analysis; 1 patient with prostatic cyst was infertile; 3 patients were detected with infravesical cysts after transurethral resection prostatectomy; 2 cases with small retention cysts were associated with benign prostatic hyperplasia; 1 case was detected with prostatic abscess; 1 patient was detected with unilateral ureterocele; 1 patient was detected with large pelvi abdominal unilateral seminal vesicle cyst; 1 patient was with unilateral ejaculatory duct cyst; 1 patient with small cystic dilation of prostatic urethra was associated with urethral stone.

Transurethral Resection Prostatectomy (TURP) and Transvesical Prostatectomy (TVP) are the two methods of operation for BPH. The aims of this study are to find out the late complication after prostatectomy between TVP dan TURP. Data were collected from January 1st 2004 to December 31st 2004 with prospective cohort study. There were 67 of 90 post prostatectomy BPH patients who met the inclusion criteria. Data were listed into age and comorbid factors are hypertension (HT), ischaemic heart disease (IHD), chronic obstructive pulmonary disease (COPD), Diabetes Mellitus (DM). Post prostatectomy late complications are erectile dysfunction (ED), retrograde ejaculation (RE), urethral stricture (US), urinary incontinence (UI). Data were analyzed statistically with the t-test. There were 67 patients (median age 65,81 years range 51-86 years) inclusion in this study 12 patients has performed TVP (17,9%), 55 patient was performed TURP (82,1%). Comorbid factors were HT (19,4%), IHD (16,4%), COPD (11,9%), DM (4,5%). The most frequent age of TVP were 7th decade (50%), TURP were 6th decade (41,82%).The late complication after TVP and TURP for BPH were not significant different (p>0,05). The most frequent late complications after prostatectomy of the TVP and TURP is ED. Study about the late complications after operation of the BPH between TVP and TURP, statistically were not significantly different.Keywords: BPH, open prostatectomy, TURP, late complication.

Hydro-electrolyte changes induced by transurethral prostatic resection

Transurethral resection prostatectomy (TURP) has been associated with severe hyponatremia due to massive absorption of bladder irrigation fluid (IF). TURP was performed in 41 patients using Sorbitol-Mannitol IF (Group A) and in 6 patients using distilled water (Group B). Six other patients were operated upon using surgical procedures identical in time and type of anesthesia to TURP (Group C). The three groups were studied with the same protocol that included blood collected before (time I), immediately after (time II) the procedure and 1 hour later (time III). Serum sodium decreased significantly in the 3 groups from time I to time II, an average of 3.4 mEq/l with Mannitol-Sorbitol, 2.3 mEq/l with distilled water, and 4.4 mEq/l in group C. Osmolality did not change significantly between the 3 times of collection and Osmolar Gap only increased from time I to II in the Sorbitol-Mannitol group. In conclusion, mild decrements in serum sodium with no clinical relevance are a common post-TURP finding, but should not be greater than in other similar general surgery without bladder irrigation. Hyposmolality did not constitute a problem.
