Patient screening prior to MR imaging: a practical approach synthesized from protocols at 15 U. S. medical centers.

The article by Boutin et al. [1] in this issue of the AJR documents a surprinsingly great variation in the MR screening procedures used at different academic centers throughout the United States. Although considerable literature now exists concerning the MR imaging of patients with ferromagnetic implants, devices, and foreign bodies [2-4], no uniform screening protocol to identify such patients has yet been adopted. The Safety Committee of the Society for Magnetic Resonance Imaging (SMRI) has recommended that "each imaging site should develop a standardized policy for screening patients with suspected metallic foreign bodies" [5]. To date, however, the SMRI has neither proposed nor endorsed a specific screening protocol for general use.

JMRI: Journal of Magnetic Resonance Imaging

<i>JMRI</i>, the official journal of the Society for Magnetic Resonance Imaging (SMRI), is produced with the assistance of the Radiological Society of North America, the largest professional organization of radiologists devoted primarily to educational endeavors. The journal describes its focus as "the appropriate use of MR in newly emerging diagnostic applications...<i>JMRI</i>is intended to provide equal opportunities for physicians, basic scientists, engineers, and imaging technologists to develop the full diagnostic potential of MR."¹The editorial board includes basic scientists and clinicians active in magnetic resonance imaging since its inception in the mid-1970s. Most represent US institutions, with fewer members from Canada, Europe, and Japan. The society selects all materials for publication, including advertisements. Detailed instructions to authors are clear and include a checklist of requirements. Cardiovascular MR imaging: current level of clinical activity.

Relative to other clinical magnetic resonance (MR) imaging activities, cardiovascular (CV) MR imaging has been slow to demonstrate a clinical presence. To better understand the present situation in clinical CV MR imaging, a survey of Society for Magnetic Resonance Imaging (SMRI) members was conducted. A large majority (78%) of the 90 sites responding to the survey reported clinical activity in CV MR imaging. Of these 70 sites, 46% restricted such activity to routine clinical work, while 3% restricted it to clinical research. The remaining 51% conducted both. At all clinical sites, the overall frequency of performance of clinical CV MR imaging was variable (mean, 4.2 and 13.8 cases per month at routine-only and combined routine-research sites, respectively). In clinical CV studies, gated static, multi-level spin-echo and dynamic gradient-echo (cine) techniques were most common. At the 68 sites involved in routine clinical CV MR imaging, primarily anatomic studies composed a much higher proportion of the total (mean, 86%) than primarily functional studies. The evaluation of acquired thoracic aortic disease, congenital cardiac malformation, and para- or intracardiac mass were the most prevalent anatomic indications overall. Approximately half of the same sites assessed functional aspects of CV disease. The assessment of ventricular dysfunction and valvular dysfunction were the most common functional objectives. The survey indicated that the level of clinical activities in CV MR imaging was low and that most responding sites were involved in clinical CV MR imaging primarily for detection and delineation of anatomic abnormalities.

An overview of digital spectrometers for MR imaging. Instrumentation Subcommittee of the SMRI Basic Science Council.

Most magnetic resonance (MR) imaging systems were originally designed with analog spectrometers, since that was the "state of the art" in the late 1970s, when they were developed. Advances in technology have allowed the design of radio-frequency electronics with a much larger percentage of digital components while the cost of such components has decreased. This has given manufacturers the incentive to develop new spectrometers that incorporate these electronics for cost reduction and potentially better performance. Upgrades and new models of MR units have become available with these so-called "digital spectrometers." Because of the interest in the new systems, the Instrumentation Subcommittee of the Basic Science Council of the Society for Magnetic Resonance Imaging has produced this report to review the basic features of analog and digital spectrometers to help the MR imaging community better understand the similarities and differences of these systems. Some details of actual commercial implementations were left out to focus on the basic features. Regardless, the authors hope they have provided a readable introduction to this important topic.

Policies, guidelines, and recommendations for MR imaging safety and patient management

The following are policies, guidelines, and recommendations from the Safety Committee of the Society for Magnetic Resonance Imaging (SMRI) concerning various issues related to magnetic resonance (MR) imaging safety and patient management. These policies, guidelines, and recommendations were developed to provide standardized and consistent information for use by health practitioners involved in clinical MR imaging.
