Emerging evidence point to a bidirectional relationship between sleep disturbances and Alzheimer's disease (AD). Poor sleep may be an overlooked risk factor for older women, who are disproportionately affected by AD and report worse subjective sleep quality than men. High genetic AD risk-characterized by the polygenic hazard score (PHS), including apolipoprotein (APOE) ε4 carriership-may further compound the effects of disrupted sleep on AD, particularly for older women. This study examined the moderating effect of genetic AD risk on subjective sleep as it related to memory and tau burden in a sample of older women. The sample consisted of older women (≥65 years old) from the Women Inflammation Tau Study. Participants completed the Pittsburgh Sleep Quality Index (PSQI), Rey Auditory Learning Test, and Brief Visuospatial Memory Test-Revised. They also underwent [18]F-MK6240 positron emission tomography. Tau burden was calculated in composite regions across Braak stages. Genetic risk groups were characterized by PHS stratified at the 75th percentile. PSQI global score × PHS group interactions on memory composite scores (N = 69) and tau burden (N = 63) were examined. PSQI global score × PHS group interactions were observed on visual memory and pathological tau in Braak regions III/IV (ps<0.10). Poorer subjective sleep was associated with worse visual memory and greater limbic tau deposition only among higher genetic risk women (ps<0.04). No significant associations were observed for verbal memory or tau in Braak regions I/II or V/VI. Older women with elevated genetic AD risk and subjective sleep difficulties may be at greater risk for visual memory deficits and tau burden in regions affected in early AD. This suggests that sleep complaints may represent a promising AD risk factor. Improving sleep may be a potential intervention target for AD mitigation and prevention, particularly for older women.

Terminal patients and their families face severe psychological distress-including anxiety, depression, caregiving stress, and grief-that exceeds the capacity of routine hospice care. While narrative nursing and psychological resilience training have shown individual efficacy, the synergistic effects of their combination remain unexamined. This study aimed to unravel the effects of narrative nursing combined with psychological resilience training in enhancing the psychological adaptation of patients and their families in hospice care. Seventy-four patients and their families receiving hospice care were allocated into an observation group (received an 8-week intervention of narrative nursing combined with psychological resilience training on top of routine hospice care) and a control group (received routine hospice care). Hospital Anxiety and Depression Scale (HADS) scores and Connor-Davidson Resilience Scale-10 (CD-RISC-10) scores of patients and their families before and after nursing were compared between the 2 groups. Psychological Distress Thermometer (PDT) and Pittsburgh Sleep Quality Index (PSQI) scores of patients before and after nursing were also compared. The modified hospice care satisfaction scale (based on the FAMCARE-2 scale) was applied to compare the nursing satisfaction of patients and their families after nursing. After nursing, the observation group demonstrated significantly greater reductions in HADS-A and HADS-D scores, more pronounced improvements in CD-RISC-10, PDT, and PSQI scores, as well as higher nursing satisfaction scores compared to the control group (all P < 0.05). The integration of narrative nursing with psychological resilience training in hospice care enhances the psychological adaptability of both patients and their families, while also contributing to improved sleep quality and greater satisfaction with care delivery.

Dexmedetomidine has evolved from a sedative adjunct to a multimodal perioperative adjunct with effects extending beyond hypnosis and analgesia. Growing interest has focused on its opioid-sparing properties, modulation of the surgical stress response, hemodynamic stabilization, organ protection, and patient-centered recovery outcomes. This review summarizes the recent clinical and translational evidence relevant to perioperative and critical care use. Contemporary meta-analyses confirm consistent reductions in perioperative opioid requirements and postoperative pain. Dexmedetomidine attenuates neuroendocrine and inflammatory stress responses and provides predictable sympatholytic hemodynamic control. Emerging high-quality evidence suggests renoprotective effects and reduced postoperative acute kidney injury, whereas cardiopulmonary and neuroprotective benefits remain less definitive. Improvements in early recovery profile, including reduced delirium, smoother emergence, and shorter ICU stay, have been described, particularly in high-risk populations. Regional and pediatric applications demonstrate prolonged analgesia and reduced emergence agitation, while ICU studies indicate benefits primarily in delirium prevention and sleep quality rather than mortality. Current evidence supports dexmedetomidine as a multimodal adjunct that enhances perioperative stability and recovery quality rather than a standalone anesthetic or outcome-modifying therapy. Careful patient selection and dose titration are essential.

Night shift work (NSW) is associated with increased cardiovascular risk, but its impact on vascular function and the role of lifestyle factors in healthy individuals remain unclear. We aimed to compare vascular function between NSW and day workers (DW) and to examine associations with objectively measured lifestyle behaviors. In this cross-sectional study, 40 healthy adults (20 DW, 20 NSW) underwent assessments of flow-mediated dilation (FMD), brachial-ankle pulse wave velocity (baPWV), and ankle-brachial index (ABI). Physical activity and sleep status were objectively measured using triaxial accelerometry for 7 consecutive days. Group comparisons and correlation analyses were conducted to evaluate the effects of NSW and its association with lifestyle factors. FMD was significantly lower in the NSW group than in the DW group (4.0% vs. 7.0%, P < 0.01), while baPWV and ABI showed no between-group differences. In the overall cohort, FMD was positively correlated with time in bed, total sleep time, and sleep efficiency, and negatively correlated with nocturnal awakening. Subgroup analysis revealed that FMD was associated with sleep quality in DW and with physical activity patterns in NSW. Chronic night shift work is associated with impaired endothelial function in healthy adults, potentially driven by disrupted sleep and reduced physical activity. Early assessment using FMD and targeted behavioral interventions may help preserve vascular health among shift workers. This study shows that healthy adults who regularly work night shifts have poorer blood vessel function than people who only work daytime hours, and that sleep and physical activity patterns may help explain this difference.

Suicide risk fluctuates rapidly, highlighting the importance of identifying risk factors for acute suicidal urges. This study examined whether nightly sleep measured actively via self-report ecological momentary assessment (EMA) and passively via wearable sensors predicted next-day suicidal urges, depression, and PTSD symptoms among military service members and veterans. Military service members and veterans with current suicidal ideation or a suicide attempt in the past month (N=86) completed seven EMA surveys per day and wore a wearable device (Fitbit) for 28days. Using multilevel models, we examined sleep assessed by EMA-only, wearable-only, and EMA+wearable as predictors of next-day suicidal urges, depression, and PTSD symptoms, and compared the predictive utility of the three approaches. Participants completed 7898 EMA observations (48.9% adherence) and wore a wearable device (Fitbit) for 79.43% of the study period. More severe nightmares and poorer sleep quality assessed by EMA predicted next next-day suicidal urges (maximum and average), suicidal beliefs, depression, and PTSD symptoms. Wearable-assessed sleep duration deviation significantly predicted next-day maximum suicidal urges. Wearable-assessed sleep regularity index predicted next-day depression. EMA-only models consistently outperformed wearable-only models in predicting next-day suicide risks and mental health outcomes, and combining EMA and wearable demonstrated best model fit. Our findings suggest that self-reported sleep via EMA has strong utility in predicting near-term suicidal risk, depression, and PTSD, while wearable devices can provide low-burden, supplemental information. Integrating wearables and EMA may enhance the prediction and inform just-in-time suicide interventions.
